Sofosbuvir, Velpatasvir, Voxilaprevir falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the honest picture for Vosevi. The interaction we have on file says that Danshen can block a liver enzyme called CYP1A2, which in theory could let drugs cleared by that enzyme build up a bit. But this concern is theoretical only. It comes from test-tube studies, and when researchers actually tested it in people using another CYP1A2 drug, no real effect showed up.

On top of that, the three medicines in Vosevi aren't mainly handled by CYP1A2, so this particular pathway is unlikely to drive a meaningful problem. The bigger reason to be careful with Danshen is its possible effect on blood clotting and the fact that it stresses your liver, which already matters when you're treating hepatitis C. Please check with your pharmacist or doctor before adding it.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

If you're on Vosevi (sofosbuvir, velpatasvir, and voxilaprevir) for hepatitis C, here's what's worth knowing. All three of these medicines rely on a transporter in your body called p-glycoprotein, which acts like a gatekeeper moving drugs in and out of cells. In a small study, Danshen slowed that gatekeeper down, raising blood levels of another medicine that uses it by roughly a third.

So in theory, Danshen could push your Vosevi levels a bit higher, which might mean more headache, tiredness, or nausea. The evidence here is limited and not specific to Vosevi, so I wouldn't panic. But since clearing this virus depends on steady drug levels, please check with your pharmacist or doctor before combining the two rather than stopping anything on your own.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

If you're taking Vosevi (sofosbuvir, velpatasvir, voxilaprevir) to clear hepatitis C, here's the concern with Danshen. When Danshen is used for a stretch of about 10 to 14 days, early human research shows it can speed up an enzyme called CYP3A4, which helps process velpatasvir and voxilaprevir. Speeding that up could lower the amount of medicine in your blood, and with hepatitis C treatment you really want steady, full levels so the virus is fully knocked out.

The worry isn't a side effect, it's that weaker drug levels could make treatment less effective. Since finishing a full, strong course matters here, please don't add Danshen without first checking with your pharmacist or doctor.

What Is Sofosbuvir, Velpatasvir, and Voxilaprevir Used For?

Brand name: Vosevi

Vosevi is used to treat chronic hepatitis C virus (HCV) infection in adults who do not have cirrhosis or who have compensated cirrhosis (mild liver scarring that is still functioning). It is specifically for people who have already been treated with an HCV regimen before and need a different option.

Vosevi is approved for adults with HCV genotype 1, 2, 3, 4, 5, or 6 who previously took a medicine that included an NS5A inhibitor. It is also approved for adults with genotype 1a or 3 who previously took sofosbuvir without an NS5A inhibitor.
