Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the gist with Myfembree (relugolix, estradiol, and norethindrone). Lab studies hint that Danshen can slow down a liver enzyme called CYP1A2, which helps process the estrogen part of this combo pill. In theory, blocking that enzyme could let estrogen build up a bit, which might mean more side effects like breast tenderness, bloating, nausea, headaches, or mood changes.

That said, this is a theoretical concern from test-tube work only. When the same effect was checked in real people using another CYP1A2 drug, it didn't actually show up. So I wouldn't lose sleep over it. Still, since Danshen can also affect blood clotting, it's worth a quick chat with your pharmacist or doctor before combining the two.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the thing with Myfembree (the relugolix, estradiol, and norethindrone combo used for fibroid or endometriosis-related symptoms): the relugolix part is moved out of your body with the help of a transporter called P-glycoprotein. Danshen seems to slow that transporter down a bit. In a small study it raised levels of another P-gp drug by around 30 to 40 percent, so the worry is that relugolix could build up a little higher than expected.

If that happens, you might notice more of relugolix's usual side effects, things like hot flashes, headaches, mood changes, or spotting. This isn't a guaranteed or dangerous interaction, and the evidence is fairly limited, but I'd still loop in your doctor or pharmacist before adding Danshen so they can keep an eye on how you feel.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Myfembree combines relugolix with two hormones, estradiol and norethindrone, and all three are broken down in part by an enzyme called CYP3A4. Here's the wrinkle with Danshen: when taken regularly for a couple of weeks, it appears to speed up CYP3A4 in the gut. That could lower the levels of your Myfembree, which may make it work less well. For someone using this medicine to manage heavy bleeding or endometriosis pain, that might mean breakthrough bleeding or less symptom relief.

There's a twist worth knowing: a single dose of Danshen seemed to do the opposite and raise drug levels. The research is early and was done with a different drug, so this is more of a heads-up than a sure thing. Please loop in your pharmacist or doctor before mixing the two.

What Is Relugolix, Estradiol, and Norethindrone Used For?

Brand name: Myfembree

Myfembree is approved for two conditions in premenopausal women: managing heavy menstrual bleeding caused by uterine fibroids (non-cancerous growths in the uterus), and managing moderate to severe pain caused by endometriosis (a condition where tissue similar to the uterine lining grows outside the uterus).

Treatment with Myfembree should not exceed 24 months because of the risk of ongoing bone loss that may not fully reverse after stopping the medication.
