CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Segesterone Acetate, Ethinyl Estradiol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
If you use Annovera (the vaginal ring with segesterone and ethinyl estradiol), here's the situation with Danshen. Part of the estrogen in your ring is processed by a liver enzyme called CYP1A2. In lab and animal studies, Danshen appears to slow that enzyme down a bit. In theory, that could let a little more estrogen build up in your body, which might mean more of the usual estrogen-related effects like nausea, breast tenderness, headaches, or bloating.
I want to be honest though: this is only a theoretical, test-tube concern. When researchers tested Danshen with a real CYP1A2 drug in people, they didn't actually see this effect. So it's worth mentioning to your pharmacist or doctor, but it's not a reason to panic. They can help you decide if Danshen is a good fit for you.

P-GLYCOPROTEIN SUBSTRATES

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Annovera is a vaginal ring that slowly releases two hormones, segesterone and ethinyl estradiol, to prevent pregnancy. The concern here is that Danshen may slow down a transporter pump called p-glycoprotein, which helps move certain drugs around your body. In one small study, Danshen raised blood levels of a p-glycoprotein medicine by roughly a third.
If the same thing happened with your ring's hormones, levels could run a little higher than usual. That might mean more of the typical hormone side effects, like nausea, breast tenderness, headaches, mood changes, or spotting. This is based on limited, lower-quality evidence, so it is a possible concern, not a sure thing. Before combining them, check with your pharmacist or doctor so they can weigh it for you.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing to keep an eye on with Annovera (segesterone acetate plus ethinyl estradiol). Both hormones in this vaginal ring are broken down partly by an enzyme called CYP3A4. In a small human study, taking Danshen for 10 to 14 days seemed to speed up that enzyme, which means it could clear these hormones out of your body faster.
Why that matters: lower hormone levels can mean weaker birth control protection, plus you might notice breakthrough bleeding or spotting. This is not a guarantee, and the evidence is still limited, but I wouldn't ignore it when reliable contraception is the goal. Please run it by your pharmacist or doctor before pairing Danshen with Annovera, since they can suggest a backup plan if needed.
