Chlorpheniramine Maleate, Dextromethorphan Hydrobromide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the gist with this cough-and-cold combo. The dextromethorphan part is partly handled by a liver and gut enzyme called CYP3A4, and Danshen can nudge that enzyme in either direction. Taken steadily for a week or two, Danshen tends to speed up CYP3A4, which could lower dextromethorphan levels and make its cough-quieting effect a little weaker. Oddly, a single dose of Danshen seems to do the opposite and may briefly raise levels.

Honestly, this is fairly minor for an over-the-counter product, and the chlorpheniramine (the antihistamine causing drowsiness and dry mouth) isn't really driven by this pathway. I wouldn't lose sleep over it, but since both Danshen and this product can leave you sleepy, run it past your pharmacist first, especially if you have heart or blood pressure concerns.

In vitro research shows that various constituents of danshen inhibit the activity of CYP2E1. So far, this interaction has not been reported in humans.

Here's the short version on combining Danshen with this cough and cold combo (the chlorpheniramine plus dextromethorphan in products like Coricidin HBP). In lab dish studies, certain compounds in Danshen can slow down a liver enzyme called CYP2E1, which is one pathway the body uses to break down some medicines. If that enzyme is blocked, a drug it handles could build up a bit and hit a little harder.

I want to be honest, though: this is purely theoretical and based only on test-tube and animal work. It has never actually been reported in real people, and neither of these ingredients is a prodrug that needs activating, so the direction wouldn't flip. In theory you might notice more drowsiness, dizziness, or dry mouth, but the evidence is weak. Still, since Danshen also affects blood clotting, it's worth a quick chat with your pharmacist or doctor before using it.
