Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for your Prempro (conjugated estrogens with medroxyprogesterone). Danshen can slow down a transporter called P-glycoprotein, which helps move certain medicines in and out of your body. In one small human study, Danshen raised blood levels of a P-glycoprotein medicine by around 30 to 40 percent. Since your hormone therapy is handled in part by this same pathway, Danshen could nudge your hormone levels a little higher than expected.

In real life, that might mean more of the usual estrogen-related effects, like breast tenderness, bloating, headaches, or nausea. This isn't a strong or well-proven interaction, so I wouldn't panic. Still, please check with your pharmacist or doctor before adding Danshen, rather than changing anything on your own.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Your Prempro combines two hormones (an estrogen and a progestin) that your body partly clears using a liver and gut enzyme called CYP3A4. Danshen seems to nudge that enzyme in different directions depending on how you take it. With regular daily use over a week or two, it appears to speed the enzyme up, which could lower your hormone levels and make the medicine work less reliably. That might mean breakthrough bleeding or a return of symptoms like hot flashes. A single dose, oddly, may briefly raise levels instead.

This is based on a small study using a different drug, so it's only a possible concern, not a sure thing. Estrogen already carries clotting risks, and Danshen may thin the blood too, so please check with your pharmacist or doctor before combining them.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

Here's the gist: your hormone medicine (the estrogen and progestin combo in Prempro) is cleared from your body partly through a process called glucuronidation, where your liver attaches a tag to the hormones so they can be flushed out. Danshen has shown, in test-tube and animal studies, that it can either speed up or slow down those same liver enzymes. That makes the direction hard to predict, and honestly, this has never actually been reported in real people.

In theory, if danshen slowed that clearance, your estrogen levels could nudge up, which might mean more breast tenderness, bloating, nausea, or spotting. If it sped things up, the hormones might wear off a bit. Because this is only lab-level evidence, I wouldn't lose sleep over it, but do mention danshen to your pharmacist or doctor before combining the two.
