Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the simple version. PMB combines conjugated estrogens with meprobamate, a calming medicine. Danshen seems to slow down a transporter in your body called P-glycoprotein, which helps move certain drugs out. When that pump works slower, those drugs can hang around a little longer and build up. In a small human study, danshen raised blood levels of one P-glycoprotein drug by about a third.

For your PMB, that could mean a touch more of the medicine sticking around than usual. The piece I'd watch most is the meprobamate part, since extra exposure could leave you feeling more drowsy, dizzy, or foggy. This is graded as a possible, moderate interaction from limited research, so it's not a sure thing. Please check with your pharmacist or doctor before adding danshen.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Your PMB tablets combine two active ingredients: conjugated estrogens (for menopause symptoms) and meprobamate (a calming, anti-anxiety medicine). Both rely partly on a liver and gut enzyme called CYP3A4 to be cleared from the body. Danshen seems to push this enzyme in different directions depending on how it's used. Taken regularly for a week or two, Danshen tends to speed up CYP3A4, which could lower your medicine levels and make them work less well. But a single dose seemed to do the opposite, raising levels briefly.

Because the direction isn't predictable, you could see weaker symptom relief, or extra drowsiness and dizziness from the meprobamate side. The evidence is only moderate, so don't panic. Please check with your pharmacist or doctor before combining them.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

Here's the situation with PMB, which combines conjugated estrogens with meprobamate (a calming, anti-anxiety ingredient). Your body clears the estrogen part partly through a process called glucuronidation, basically tagging the hormone so you can flush it out. In lab dishes, Danshen has shown a mixed effect on those tagging enzymes, sometimes slowing them down and sometimes speeding them up. If they slow down, estrogen could linger a bit longer, which in theory might bump up side effects like breast tenderness, bloating, nausea, or spotting.

The honest part: this has only been seen in test tubes and animals, never reported in actual people, so it's very theoretical. I wouldn't lose sleep over it, but since Danshen can also thin the blood, it's worth a quick chat with your pharmacist or doctor before combining the two.
