Acetaminophen, Chlorzoxazone, Codeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the gist for this combo pill. Lab and animal studies hint that Danshen may slow down the CYP1A2 enzyme, one of the liver's drug-processing pathways. If that happened in real life, certain ingredients cleared by CYP1A2 could build up a bit, in theory leading to more drowsiness, dizziness, or stomach upset. Keep in mind the codeine part is a prodrug, but it's activated mainly by a different enzyme (CYP2D6), so this particular pathway isn't its main route.

The honest truth: this is theoretical. When researchers tested Danshen with a similar CYP1A2 drug in actual people, they didn't see a meaningful effect. Still, since Danshen can also affect bleeding and clotting, please run it by your pharmacist or doctor before combining the two.

In vitro research shows that various constituents of danshen inhibit the activity of CYP2E1. So far, this interaction has not been reported in humans.

This combo pill has three ingredients, and the part that matters most here is the muscle relaxer chlorzoxazone, which your body clears using an enzyme called CYP2E1. In lab dishes, danshen seems to slow that enzyme down. If that happened in real life, chlorzoxazone could build up a bit, which might mean more drowsiness, dizziness, lightheadedness, or stomach upset. The acetaminophen and codeine pieces aren't the main concern with this particular pathway.

I want to be honest about the strength of this: it's theoretical, based only on test-tube and animal studies, and it hasn't actually been reported in people. So I wouldn't panic. Still, danshen can also affect blood clotting, so please run it by your pharmacist or doctor before combining the two.

Acetaminophen, Chlorzoxazone, Codeine also falls under GLUCURONIDATED DRUGS.

Theoretically, danshen might affect the levels and clinical effects of drugs requiring glucuronidation.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

This combination pill bundles three medicines, and the part that overlaps with Danshen is mostly the acetaminophen, which your body clears largely through a process called glucuronidation. In a test tube, Danshen can both speed up and slow down those clearing enzymes, so the lab signals point in opposite directions. The honest takeaway is that nobody really knows the net effect in real people, since this has never actually been reported in humans.

In theory, if clearance slowed, acetaminophen levels could drift higher, which matters because too much acetaminophen is hard on the liver. Codeine also adds the usual concerns like drowsiness and constipation. Because the evidence here is only lab-based and weak, I would not panic, but it is worth a quick chat with your pharmacist before pairing them.
