Why this applies to Crizotinib

Crizotinib falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for crizotinib (Xalkori): it relies partly on a cellular pump called P-glycoprotein to move it out of your body. Danshen can slow that pump down. In one study, danshen raised levels of another pump-dependent drug by about 30 to 40 percent. So in theory, danshen could let more crizotinib build up in your system than expected.

With crizotinib, higher levels can mean more of its known side effects, like nausea, vision changes, swelling, fatigue, or effects on your heart rhythm and liver. This is a moderate, possible interaction based on limited human data, so it's not a sure thing, but crizotinib is a serious cancer medicine where I'd rather be careful. Please don't add or stop danshen on your own. Check with your oncologist or pharmacist first.

Crizotinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing with Danshen and crizotinib (Xalkori). Your body relies on an enzyme called CYP3A4 to break down crizotinib, and Danshen seems to nudge that enzyme in different directions depending on how you take it. In the research, taking Danshen daily for a week or two actually sped up clearance of a CYP3A4 drug, which could lower crizotinib levels and, in theory, make it work less well against the cancer. A single dose did the opposite and pushed levels up.

Because this is a cancer medicine where consistent blood levels really matter, I wouldn't want it swinging either way. Please don't start or stop anything on your own. Run Danshen past your oncologist or pharmacist first so they can keep your treatment on track.

About Crizotinib

What Is Crizotinib Used For?

Brand name: Xalkori

Xalkori (crizotinib) is used to treat adult patients with metastatic non-small cell lung cancer (NSCLC) whose tumors test positive for the ALK or ROS1 gene abnormality, as confirmed by an FDA-approved test.

Xalkori is also used to treat pediatric patients aged 1 year and older and young adults with relapsed or refractory systemic ALK-positive anaplastic large cell lymphoma (ALCL). Additionally, it treats adult and pediatric patients aged 1 year and older with unresectable, recurrent, or refractory ALK-positive inflammatory myofibroblastic tumor (IMT). Note: Xalkori's safety and effectiveness in older adults with relapsed or refractory ALK-positive ALCL have not been established.
