Caffeine (prescription drug) falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the gist: your body clears caffeine using a liver enzyme called CYP1A2. In test-tube studies, Danshen seems to slow that enzyme down. If that happened in real life, caffeine could hang around longer and hit harder, meaning you might feel more jittery, wired, anxious, have a racing heart, or trouble sleeping.

The good news is this is mostly theoretical. It showed up in lab dishes, not in people. In fact, a human study with a similar CYP1A2 drug (theophylline) didn't show a real problem. So I wouldn't panic. Still, if you notice extra caffeine-type side effects after starting Danshen, dial back your coffee and check in with your pharmacist or doctor.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the practical picture for prescription caffeine. The interaction on file is about Danshen affecting CYP3A4, an enzyme in your gut and liver. The research showed that taking Danshen for 10 to 14 days actually speeds up that enzyme, which can lower the levels of drugs it processes. A single dose, on the other hand, briefly slowed things down and raised drug levels. So the direction depends on how long you've been using it.

One honest caveat: caffeine is mostly handled by a different enzyme (CYP1A2), not CYP3A4, so this particular pathway may not move your caffeine levels much. Still, if you notice more jitteriness, a racing heart, or trouble sleeping, or the opposite (less effect), check in with your pharmacist or doctor before changing anything.

What Is Caffeine Used For?

Brand names: Dextrophin, Caffeine

Caffeine citrate injection and oral solution are used for the short-term treatment of apnea of prematurity. This means pauses in breathing in premature infants. The label lists the age range as infants between 28 weeks and a later gestational age, and that range is cut off in the text provided.

Over-the-counter caffeine tablets, such as Diurex and the Stay Awake products, are taken by mouth by adults and older children. Their labels point to fatigue and drowsiness. Diurex directions also mention taking it while symptoms persist.
