CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Acetaminophen, Dichloralantipyrine, Isometheptene falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination medicine (used for migraines) contains acetaminophen, a mild sedative, and isometheptene to ease blood vessel-related headache pain. The concern here is that Danshen may block a liver enzyme called CYP1A2, which helps clear some of these ingredients. If that enzyme slows down, the drug could build up a bit and you might feel more drowsy or lightheaded than usual.
Honestly though, this is mostly theoretical. It comes from lab studies, and when researchers tested it in real people using another CYP1A2 drug, they didn't see a real effect. So I wouldn't lose sleep over it, but since Danshen can also affect blood clotting, it's worth a quick chat with your pharmacist before combining them.

CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES

In vitro research shows that various constituents of danshen inhibit the activity of CYP2E1. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination headache product contains acetaminophen (Tylenol), which is the piece that matters here. A small share of acetaminophen is handled by a liver enzyme called CYP2E1. In lab dish studies, certain compounds in Danshen appear to slow that enzyme down. In theory, that could let a bit more acetaminophen linger in your system. The catch is that this has only been seen in test tubes and animals, never reported in real people, so I wouldn't lose sleep over it.
The realistic worry with acetaminophen is liver stress if levels run high, so I'd avoid going overboard. Before pairing Danshen with this medicine, run it by your pharmacist or doctor, especially since Danshen can also affect blood clotting.

GLUCURONIDATED DRUGS
3 of 3Acetaminophen, Dichloralantipyrine, Isometheptene also falls under GLUCURONIDATED DRUGS.
In vitro research shows that danshen induces the expression of glucuronosyltransferases. However, it also inhibits the activity of glucuronosyltransferases, including various members of the 1A and 2B families. The extent of inhibition of a specific glucuronosyltransferase seems to be dependent on whether or not the danshen is processed via 'sweating'. This type of processing may affect the levels of constituents in danshen that alter glucuronosyltransferase activity. So far, this interaction has not been reported in humans.

This combination medicine is mostly known for its acetaminophen (Tylenol) component, which your liver clears largely by attaching a sugar molecule to it (a process called glucuronidation). Danshen has been shown in lab and animal studies to tug on those same glucuronidation enzymes, sometimes speeding them up and sometimes slowing them down. Which way it leans seems to depend on how the danshen was processed, so the picture is genuinely murky.
The honest takeaway: this is a theoretical interaction that has not been reported in real people. If the enzymes were blocked, acetaminophen could linger a bit longer, which matters since high acetaminophen levels stress the liver. I wouldn't lose sleep over it, but please run it past your pharmacist or doctor before pairing them.
