Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
Why this applies to Chlorpheniramine Maleate, Dextromethorphan Hydrobromide
Chlorpheniramine Maleate, Dextromethorphan Hydrobromide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Indian Snakeroot interacts with.
Indian snakeroot contains small amounts of the drug yohimbine. In vitro research shows that yohimbine inhibits CYP2D6 enzyme activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo. Both the chlorpheniramine and the dextromethorphan in your cough and cold product are cleared by a liver enzyme called CYP2D6. Indian snakeroot may slow that enzyme down a bit (this is only a lab-based, theoretical concern from a small amount of yohimbine in the plant, not something proven in people). If it does slow it, these ingredients could linger longer in your body and hit harder.
In real life that might mean more drowsiness, dizziness, dry mouth, or feeling foggy, and with extra dextromethorphan, possible restlessness or, rarely, confusion. On top of that, Indian snakeroot lowers blood pressure on its own, so adding it can leave you lightheaded. Please don't start or stop anything on your own. Check with your pharmacist or doctor first.

From our pharmacist’s overview on the Chlorpheniramine Maleate, Dextromethorphan Hydrobromide page
“Grapefruit is the interaction I would flag first for anyone taking chlorpheniramine with dextromethorphan (Coricidin HBP Cough & Cold). It blocks the enzyme that clears these ingredients, so levels and side effects can climb.”

About Indian Snakeroot
What is Indian Snakeroot?
- High blood pressure
- Anxiety and stress
- Insomnia
- Agitation
- Fever (traditional use)
Indian snakeroot is a traditional Ayurvedic plant that contains reserpine, a compound with real blood-pressure-lowering and sedative effects. Because its potent alkaloids can cause serious side effects like depression and very low blood pressure, it should never be used without medical supervision. It is not a safe or simple home remedy, and many countries restrict its use.
