Mommy's Bliss Gripe Water and Risperidone

Here is the interaction we found between these two, with the evidence behind it.

Minor

Cytochrome P450 3A4 (Cyp3A4) Substrates

Why this applies to Risperidone

Risperidone falls under Cytochrome P450 3A4 (Cyp3A4) Substrates — the group Mommy's Bliss Gripe Water interacts with.

Mommy's Bliss Gripe Water contains fennel seed. Theoretically, fennel might increase levels of drugs metabolized by CYP3A4. However, this has only been seen in vitro (i.e., in a lab setting) and has not been reported in humans. Additionally, the amount of fennel seed contained in Mommy's Bliss Gripe water is very low, and clinically significant interactions here are thought to be unlikely.
Severity Minor Likelihood Unlikely Evidence C

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: the fennel in this gripe water can, at least in a lab dish, slow down an enzyme called CYP3A4. Your body uses that enzyme to help break down risperidone. In theory, blocking it could let risperidone build up a bit, which might mean more of its usual effects like drowsiness, dizziness, low blood pressure when standing, or restlessness.

The reassuring part is that this has only been seen in a test tube, never in real people, and the amount of fennel in gripe water is tiny. So a meaningful interaction here is considered unlikely. Since risperidone is often used in kids, I'd still loop in your pediatrician or pharmacist before combining the two, just to be safe.
