Irinotecan falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Theaflavin interacts with.

In vitro, theaflavin inhibits organic anion-transporting polypeptide (OATP)2B1. OATP2B1 is expressed in the small intestine and liver and is responsible for the uptake of drugs and other compounds. This interaction has not been shown in humans. However, theaflavin might reduce the absorption of drugs that are OATP2B1 substrates. Use with caution.

Here's the gist for irinotecan (Camptosar). Theaflavins, the antioxidants found in black tea, were shown in a lab dish to block a transporter called OATP2B1 that helps move certain drugs into your body. In theory, if that transporter is slowed, your body might take up a bit less of an affected medication. Important to know: this has only been seen in test tubes and animals, never in actual people, so it's a possible concern, not a proven one.

Irinotecan is a powerful chemotherapy drug, and it's given by IV in a carefully controlled setting, so a tea-based effect on gut absorption is unlikely to change much. Still, because this is cancer treatment, don't guess. Please run any black tea extracts or concentrated theaflavin supplements by your oncologist or pharmacist first.
