Shiitake mushroom as a cooked food is generally well tolerated. Concentrated supplements like this one should be used with caution because safety data at supplement doses is limited.
The most common side effects when taken orally are abdominal discomfort, bloating, diarrhea, nausea, and vomiting. Raw shiitake can cause a distinctive rash called shiitake dermatitis and rarely intestinal blockage.
People who handle shiitake extensively may develop allergic reactions including asthma, skin rashes, and in severe cases, hypersensitivity pneumonitis (a lung condition from spore inhalation).

Formulation
A standardized cultured extract of medicinal mushroom mycelia (roots), AHCC is one of the world's most researched specialty immune supplements, supported by over 30 human studies. AHCC has been clinically shown to maintain optimal NK cell, T-cell, macrophage, and dendritic cell activity. Unlike most other medicinal mushroom extracts, AHCC has a very low molecular weight of just 5,000 daltons and is rich in alpha-glucans - increasing absorption and efficacy. Mycelia extract of hybridized basidiomycete (Lentinula edodes) mushrooms
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.
Immunosuppressants
Theoretically, taking shiitake mushroom might decrease the effects of immunosuppressive therapy.
In vitro evidence suggests that shiitake mushroom extracts stimulate immune function.
