Does it work?
Moderate evidence
Beta-sitosterol and sitosterol (which are the same compound in different naming conventions) are likely effective for benign prostatic hyperplasia (BPH) — enlargement of the prostate gland — according to clinical evidence. They're possibly effective for familial high cholesterol and heart disease risk.
The strength of evidence for cholesterol and heart health is "possibly effective," meaning there's some support but it's not as robust as the BPH data.
How safe is it?
Well-documented data
Beta-sitosterol and sitosterol are generally well tolerated in the short term, though you should check with your doctor before starting. The most common side effects are digestive: constipation, diarrhea, gas, indigestion, and nausea.
A small number of people report reduced appetite. There's a rare case report of acute inflammation of the pancreas (pancreatitis) that started on the first day of use and resolved when the person stopped — this is not a common occurrence.
There isn't enough safety data to use this product during pregnancy or while breastfeeding, so talk with your doctor or pharmacist if you're in either situation.
