The evidence for D-mannose is mixed. It's rated likely effective for carbohydrate-deficient glycoprotein syndrome type 1b, a rare genetic condition.
For urinary tract infections (UTIs), though, it's rated possibly ineffective — meaning the current evidence doesn't support its use for that purpose, even though it's widely marketed for it.
How safe is it?

D-mannose is generally well tolerated in the short term, though long-term safety data at high doses are limited. The most common side effects are bloating, diarrhea, and nausea.
Diarrhea is more likely if you take a single dose larger than 200 mg per kilogram of body weight. Not enough reliable safety data exist for pregnancy — talk with your doctor or pharmacist before using it if you're pregnant.
