Bromelain is generally well tolerated for short-term use, but long-term safety data is limited. The most common side effects when taken by mouth are diarrhea, flatulence, upset stomach, and headache.

Rarely, people allergic to bromelain may have IgE-mediated allergic reactions (hives, itching), and inhaling it can trigger asthma or throat symptoms. Protease is usually well tolerated but may cause digestive upset or allergic reactions in some people.

Biotin is generally considered safe even at high doses because your body excretes excess in urine; a few people taking very high doses (300 mg daily) reported mild diarrhea. For pregnancy, bromelain safety data is insufficient — it's best avoided.

Biotin from food and prenatal vitamins is appropriate, but high-dose supplements should be discussed with your doctor first. During breastfeeding, bromelain and protease safety is not well established — avoid without medical advice — while biotin from normal dietary and supplement amounts is likely fine, though discuss high-dose products with your doctor.

Recommended use: Take contents of one Enzyme Packet three times daily on an empty stomach.

Anticoagulant/Antiplatelet Drugs

Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. However, observational research has found that topical bromelain debridement is not associated with increases or decreases in laboratory markers of coagulation when compared with surgical debridement.

Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
