The evidence supports Tinospora cordifolia for type 2 diabetes—clinical research shows it can lower fasting blood sugar and glycated hemoglobin (a measure of long-term blood sugar control), making it possibly effective for that use. When taken by mouth, Tinospora cordifolia is generally well tolerated, though headache and nasal pain have been reported in trials. The chief concern is liver injury—rare, but serious.
Case reports describe autoimmune hepatitis, acute hepatitis, worsening of existing liver disease, and acute liver failure in 49 patients after 42–90 days of use; 2 required transplant and 4 died. Because of this risk, the safety data advises against use in pregnancy and while breastfeeding.

The extract also theoretically inhibits several liver enzymes (CYP2D6, CYP2C19, CYP1A2, and CYP2C9) that break down many medications. In lab studies, these inhibitions occur at high concentrations, and in one animal study, the extract reduced clearance of a CYP2C9 substrate.
This could raise blood levels of drugs metabolized by these pathways—though these interactions have not been confirmed in people. Additionally, Tinospora cordifolia has immune-stimulating properties that could theoretically reduce the effectiveness of immunosuppressants (medications given after organ transplant or for severe autoimmune conditions).

Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, Tinospora cordifolia might increase levels of drugs metabolized by CYP1A2.
In vitro research shows that Tinospora cordifolia extract inhibits CYP1A2 at high concentrations. However, this interaction has not been reported in humans.
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, Tinospora cordifolia might increase levels of drugs metabolized by CYP2C19.
In vitro research shows that Tinospora cordifolia extract inhibits CYP2C19 at high concentrations. However, this interaction has not been reported in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, Tinospora cordifolia might increase levels of drugs metabolized by CYP2C9.
In vitro research shows that Tinospora cordifolia extract inhibits CYP2C9. Animal research shows that Tinospora cordifolia extract 400 mg/kg twice daily for 14 days reduces the clearance and increases plasma levels of glyburide, a CYP2C9 substrate. However, this interaction has not been reported in humans.
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, Tinospora cordifolia might increase levels of drugs metabolized by CYP2D6.
In vitro research shows that Tinospora cordifolia extract inhibits CYP2D6 at high concentrations. However, this interaction has not been reported in humans.
Immunosuppressants
Theoretically, Tinospora cordifolia might reduce the effectiveness of immunosuppressants.
In vitro and animal research shows that Tinospora cordifolia has immunostimulant effects.
