The other ingredients — gardenia, codonopsis, bupleurum, dong quai, and Chinese peony — all lack reliable evidence for the conditions they're traditionally used for. We don't hold effectiveness data for oyster extract or alisma.

How safe is it?
Well-documented data
Most of these ingredients are generally well tolerated when used short-term, though high-quality safety data are limited for several of them. Gardenia carries caution because large or long-term amounts haven't been well studied, and orally it's been linked in one case to blue-gray skin pigmentation after 7 years of use.
Bupleurum has been rarely linked to lung and liver problems in combination products, though it's unclear whether bupleurum or other ingredients were responsible. Cyperus rhizome extract is used very cautiously because human safety data are minimal.
Codonopsis can cause allergic reactions including anaphylaxis, and very large doses (30–60 grams) may cause chest pain, arrhythmia, throat pain, or loss of voice. Dong quai is generally well tolerated orally but may increase your skin's sensitivity to sunlight.
Peony can cause gastrointestinal upset — diarrhea was reported in 5% of patients taking high doses in one study. Regarding pregnancy and breastfeeding, the safety data advises against gardenia, codonopsis, bupleurum, isatis root extract, isatis leaf extract, and cyperus rhizome extract during both pregnancy and lactation.
Dong quai is rated possibly unsafe in pregnancy. Chinese peony is also rated possibly unsafe in pregnancy.

The most serious interaction is dong quai with warfarin (a blood thinner), where case reports show increased bleeding risk and elevated anticoagulant effects — a Major severity concern.

Dong quai, codonopsis, bupleurum, and Chinese peony all interact with blood thinners and antiplatelet drugs (Moderate severity), increasing bleeding risk. Codonopsis and bupleurum can theoretically increase the effects of diabetes medications, raising the risk of low blood sugar (hypoglycemia).
Bupleurum may reduce the effectiveness of immunosuppressants. Chinese peony can increase levels of certain drugs metabolized by your liver (CYP1A2 and CYP3A4 substrates, including clozapine), interfere with estrogen-based hormone therapy and birth control pills, and theoretically lower phenytoin levels.
Dong quai may also interfere with estrogen therapy. Cyperus rhizome extract can increase drowsiness when combined with sedating medications like benzodiazepines, barbiturates, and other CNS depressants (Moderate severity).
Gardenia theoretically increases the effects of stimulant laxatives (Moderate severity).

Chinese Peony Root Extract
Anticoagulant/Antiplatelet Drugs
Theoretically, combining peony with anticoagulant or antiplatelet drugs might increase the risk of bleeding.
In vitro research suggests that peony might have antiplatelet, anticoagulant, and antithrombotic effects.
Clozapine (Clozaril)
Theoretically, peony might increase the levels and clinical effects of clozapine.
In vitro research shows that peony suppresses the metabolism of clozapine via weak-to-moderate inhibitory effects on cytochromes P450 (CYP) 1A2 and CYP3A4. This effect has not been reported in humans.
Contraceptive Drugs
Theoretically, peony might interfere with contraceptive drugs due to competition for estrogen receptors.
In vitro and animal research shows that peony extract has estrogenic activity. Concomitant use might also increase the risk for estrogen-related adverse effects.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, use of peony may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that peony suppresses the metabolism of clozapine via weak-to-moderate inhibitory effects on CYP1A2 and CYP3A4. This effect has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, use of peony may increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that peony suppresses the metabolism of clozapine via weak-to-moderate inhibitory effects on CYP1A2 and CYP3A4. This effect has not been reported in humans.
Estrogens
Theoretically, concomitant use of large amounts of peony might interfere with hormone replacement therapy and/or increase the risk for estrogen-related adverse effects.
In vitro and animal research shows that peony extract has estrogenic activity. Theoretically, peony might compete for estrogen receptors and/or cause additive estrogenic effects.
Phenytoin (Dilantin)
Theoretically, peony might reduce the levels and clinical effects of phenytoin.
Animal research shows that taking peony root reduces levels of phenytoin. Some researchers suggest that peony root might affect cytochrome P450 (CYP) 2C9, which metabolizes phenytoin. However, preliminary research in humans shows that peony root does not alter levels of losartan (Cozaar), which is also metabolized by CYP2C9.
Bupleurum
Anticoagulant/Antiplatelet Drugs
Theoretically, bupleurum might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
In vitro research suggests that saikosaponins, constituents of bupleurum, can inhibit platelet aggregation.
Antidiabetes Drugs
Theoretically, bupleurum might decrease the effects of antidiabetes drugs.
Animal research suggests that saikosaponins, constituents of bupleurum, can increase blood glucose.
Immunosuppressants
Theoretically, bupleurum might decrease the effects of immunosuppressants.
In vitro and animal research suggests that bupleurum might stimulate immune function.
Cyperus Rhizome Extract
Barbiturates
Animal research suggests that taking adrue in combination with sodium thiopental increases total sleep time three-fold compared to the effects of sodium thiopental alone. Theoretically, concomitant use of adrue and barbiturates might increase the risk of drowsiness and motor reflex depression. Some barbiturates include amobarbital (Amytal), butabarbital (Butisol), mephobarbital (Mebaral), pentobarbital (Nembutal), phenobarbital (Luminal), secobarbital (Seconal), and others.
Benzodiazepines
Animal research suggests that taking adrue in combination with diazepam increases total sleep time four-fold compared to the effects of diazepam alone. Theoretically, concomitant use adrue and benzodiazepines might increase the risk of drowsiness and motor reflex depression. Some benzodiazepines include clonazepam (Klonopin), diazepam (Valium), lorazepam (Ativan), and others.
Cns Depressants
Animal research suggests that taking adrue in combination with sodium thiopental or diazepam increases total sleep time up to four-fold compared to the effects of the drugs alone. Theoretically, concomitant use of adrue with CNS depressants might cause additive sedation. Some CNS depressants include benzodiazepines, such as diazepam (Valium), alprazolam (Xanax), triazolam (Halcion), and estazolam (ProSom); barbiturates, such as mephobarbital (Mebaral), phenobarbital (Luminal Sodium), and pentobarbital sodium (Nembutal); zolpidem (Ambien); and others.
Codonopsis root extract
Abiraterone (Zytiga)
Theoretically, taking codonopsis root with abiraterone might reduce the levels and therapeutic effects of abiraterone.
Animal research in rats shows that intragastric administration of codonopsis root along with abiraterone every 2 days for 2 weeks seems to increase the clearance of abiraterone and reduce the overall exposure and time to maximum concentration. This interaction has not been reported in humans.
Anticoagulant/Antiplatelet Drugs
Theoretically, codonopsis liquor might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
A small clinical study in adults with coronary heart disease shows that consuming Codonopsis pilosula liquor for 4 weeks inhibits platelet aggregation but does not affect tissue-type plasminogen activator (t-PA) or plasminogen activator inhibitor (PAI).
Antidiabetes Drugs
Theoretically, codonopsis might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Laboratory and animal research suggest that codonopsis has antidiabetic effects.
Dong Quai
Warfarin (Coumadin)
Dong quai may increase the risk of bleeding when used with warfarin.
Case reports suggest that concomitant use of dong quai with warfarin can increase the anticoagulant effects of warfarin and increase the risk of bleeding. In one case, after 4 weeks of taking dong quai 565 mg once or twice daily, the international normalized ratio (INR) increased to 4.9. The INR normalized 4 weeks after discontinuation of dong quai.
Anticoagulant/Antiplatelet Drugs
Theoretically, dong quai may increase the risk of bleeding when used with anticoagulant or antiplatelet drugs; however, research is conflicting.
Animal studies suggest that dong quai has antithrombin activity and inhibits platelet aggregation due to its coumarin components. Additionally, some case reports in humans suggest that dong quai can increase the anticoagulant effects of warfarin. However, clinical research in healthy adults shows that taking 1 gram of dong quai root daily for 3 weeks does not significantly inhibit platelet aggregation or cause bleeding. Until more is known, use dong quai with caution in patients taking antiplatelet/anticoagulant drugs.
Estrogens
Theoretically, dong quai may reduce the effects of estrogens.
Dong quai has estrogenic effects. Theoretically, concomitant use of large amounts of dong quai might interfere with hormone replacement therapy due to competition for estrogen receptors.
Gardenia
Stimulant Laxatives
Theoretically, gardenia might increase the effects and adverse effects of stimulant laxatives.
Animal research shows that geniposide, a glucoside found in gardenia fruit, may function as a laxative and cause diarrhea when taken orally.
