L-Carnitine Ingredients & Drug Interactions
by Thorne
What is this page for?
First and foremost: checking L-Carnitine against your medications. The heart of this page is the interaction checker and the full interaction report — how this product’s ingredients may interact with prescription and over-the-counter medicines you may be taking.
Around that, we add a pharmacist’s high-level view of the product as a whole — what’s inside, the evidence for its stated use, how transparent the label is, and what safety data exists — so you can see the full picture in one place. It’s educational information from our licensed clinical databases and the clinical staff at HelloPharmacist — not medical advice — and we don’t sell or endorse products. Our editorial policy

HelloPharmacist Scorecard of L-Carnitine by Thorne
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: L-carnitine, a compound your body naturally produces and uses to move fatty acids into cells so they can be burned for energy. The remaining ingredients — hypromellose, leucine, and silicon dioxide — are inactive components that form the capsule and help with tablet integrity and absorption.
Does it work?
Moderate evidence
L-Carnitine is effective for treating L-carnitine deficiency. It's possibly effective for high cholesterol (hyperlipidemia), congestive heart failure (CHF), and angina.
The evidence supporting use for these other conditions is not yet conclusive, so talk with your doctor about whether it's right for your specific situation.
How safe is it?
Well-documented data
L-Carnitine is generally well tolerated at typical doses. Common side effects include stomach cramps, abdominal pain, diarrhea, nausea, heartburn, and reduced appetite — some people also report a fish-like body odor.
High doses are more likely to cause digestive upset. Seizures have been reported rarely.
The safety data during breastfeeding is limited, so caution is advised if you're nursing — talk with your doctor or pharmacist about whether it's appropriate for you.
Meds to double-check
Moderate interaction found
Check with your doctor or pharmacist before taking L-Carnitine if you use warfarin or acenocoumarol (blood thinners) — L-Carnitine may increase their anticoagulant effects. Also flag thyroid hormone replacement (such as levothyroxine) — L-Carnitine may reduce its effectiveness.
Use the tool below to search your exact medications.
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence for its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
L-Carnitine may help if you have a documented carnitine deficiency or are exploring support for heart or cholesterol issues — but it's a strong candidate for a medication check before you start. If you take warfarin, acenocoumarol, or any thyroid hormone, speak with your doctor or pharmacist first; the interactions are real and could affect how well your medications work.
Expect mild digestive side effects and the possibility of a fishy odor at higher doses.

L-Carnitine by Thorne Drug Interactions
HelloPharmacist Interaction Report
L-Carnitine by Thorne interacts with blood thinners (anticoagulants) and thyroid hormone replacement — the most serious concern is a Moderate-severity interaction with warfarin and acenocoumarol, where L-carnitine may strengthen their anticoagulant effects.
There are at least two case reports of elevated INR (a measure of how thin your blood is) when L-carnitine was used alongside acenocoumarol.
Read the full breakdown — every affected drug type, severity by severity
L-Carnitine may also decrease how well thyroid hormone replacement works by acting as a peripheral antagonist — essentially blocking thyroid hormone from entering cells where it's needed. If you take thyroid medication, this could reduce its effectiveness and potentially worsen your thyroid condition.
Altogether, these interactions span 19 individual medications. Use the interaction checker below to verify your exact prescriptions before starting this supplement, and talk with your doctor or pharmacist before combining L-Carnitine with any blood thinner or thyroid hormone.

For each ingredient in L-Carnitine with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
