Mugwort is generally well tolerated, but a few things matter. People sensitive to ragweed or related plants can have allergic reactions—symptoms have ranged from asthma, runny nose, and eye irritation to rash and, rarely, severe anaphylaxis.
There's also a report of mania in one person who consumed a very large amount of mugwort infusion; the cause was linked to thujone, a compound in mugwort. While thujone levels are normally low, they can vary between batches.
Mugwort should not be used during pregnancy—the safety data advises against it because it's traditionally been used to stimulate the uterus. There isn't enough safety information for breastfeeding, so it's best avoided then too.
