HelloPharmacist Scorecard of Marshmallow by Hawaii Pharm
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: marshmallow leaf extract in a dried form. Marshmallow leaf has been used traditionally to soothe the respiratory tract and skin, though the evidence for these uses is still being established.
The liquid also holds alcohol, water, and glycerin as inactive ingredients that preserve and deliver the extract.
Does it work?
Couldn't assess
The data we hold shows insufficient reliable evidence to rate marshmallow leaf for the conditions it's been studied in—vaginal yeast infections, diarrhea, leg ulcers, eczema, and respiratory infections. That doesn't mean it doesn't work; it means large, rigorous human trials haven't yet confirmed its benefits for these uses.
If you're considering it for a specific health concern, talk it over with your doctor or pharmacist about what the evidence actually supports.
How safe is it?
Some data gaps
Marshmallow leaf is generally well tolerated for short-term use, though human safety data are limited. The product should be avoided during pregnancy and while breastfeeding—there isn't enough reliable safety information to know it's safe in these situations.
If you're pregnant, breastfeeding, or planning to be, check with your doctor or pharmacist before taking this.
Meds to double-check
Moderate interaction found
Double-check this product before you start if you take lithium—marshmallow may raise lithium levels in your blood, and your dose might need adjusting. Also flag any oral medications you take regularly, since marshmallow's mucilage can interfere with how your body absorbs them.
If you take anticoagulants or antiplatelet drugs, mention this product to your pharmacist or doctor as well.
The bottom line
Scorecard at a glanceFully disclosed formula with no assessable stated purpose. Moderate medication interactions have been identified, and some safety-information gaps remain.
Marshmallow leaf extract is generally well tolerated but interacts with lithium and can affect how your body absorbs other oral medications. If you take lithium, blood thinners, or any regular oral drugs, clear this product with your doctor or pharmacist first.
It's not recommended in pregnancy or while breastfeeding. Talk with your healthcare provider about whether it's right for your situation.

Marshmallow by Hawaii Pharm Drug Interactions
HelloPharmacist Interaction Report
Hawaii Pharm Marshmallow Leaf Extract can interact with medications, chiefly through its potential diuretic and mucilage content.
The most serious concern is with lithium (a mood-stabilizing medication): marshmallow's diuretic effects may reduce how your body clears lithium, raising levels in your blood and risking toxicity. Your doctor may need to adjust your lithium dose if you add this product.
Read the full breakdown — every affected drug type, severity by severity
Marshmallow's mucilage can also slow or block how your body absorbs oral medications taken by mouth. If you take any oral drugs, space them 30–60 minutes apart from marshmallow to minimize this effect.
Additionally, marshmallow flower (though less common in supplements than the leaf in this product) may have mild blood-thinning properties; animal data suggest a theoretical concern with anticoagulants or antiplatelet drugs, though evidence in humans is limited.
Before you start, run your exact medications through the interaction checker below—especially if you take lithium, blood thinners, or any oral prescriptions.

For each ingredient in Marshmallow with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
Marshmallow Leaf Extract, Dry
Lithium
Theoretically, due to potential diuretic effects, marshmallow might reduce excretion and increase levels of lithium.
Marshmallow is thought to have diuretic properties. To avoid lithium toxicity, the dose of lithium might need to be decreased when used with marshmallow.
Anticoagulant/Antiplatelet Drugs
Theoretically, marshmallow flower might have antiplatelet effects.
Animal research suggests that marshmallow flower extract has antiplatelet effects. However, the root and leaf of marshmallow, not the flower, are the plant parts most commonly found in dietary supplements. Theoretically, use of marshmallow flower with anticoagulant/antiplatelet drugs can have additive effects, and might increase the risk for bleeding in some patients.
Oral Drugs
Theoretically, mucilage in marshmallow might impair absorption of oral drugs.
Marshmallow contains mucilage which can affect oral drug absorption. To avoid changes in absorption, take marshmallow 30-60 minutes after oral medications.
