Maxi-Zyme Caps contains 8 active ingredients, all enzymes that help break down different food components. Papain and bromelain are proteases — they break down proteins.
Protease is another protein-digesting enzyme. Lipase breaks down fats, alpha-amylase breaks down carbohydrates, and phytase helps release minerals from plant foods.
Lactase breaks down milk sugar (lactose). The product also includes alpha-galactosidase, which breaks down complex sugars that can cause gas.
Two inactive ingredients — maltodextrin and cellulose — are fillers and binders that give the capsule its form.
Does it work?
Strong evidence
The evidence for most of these enzymes is not established in the data we hold. Lactase is the one exception: it's rated effective for lactose intolerance — the inability to digest milk sugar.
For papain, bromelain, protease, lipase, phytase, and alpha-amylase, the available research does not support claims about cancer, muscle soreness, wounds, diabetes, digestive upset, or other conditions. The evidence is simply insufficient to rate how well they work.
How safe is it?
Well-documented data
Papain and bromelain are generally well tolerated when taken short-term at normal doses, though both can cause digestive upset, allergic reactions, and headache in some people. High doses of papain may be unsafe and could rarely cause severe stomach irritation or esophageal damage.
Protease, lactase, lipase, and phytase are also generally well tolerated but may trigger allergic reactions in sensitive individuals. Some people who work with airborne enzyme dust develop occupational allergies; this is not a concern with taking the capsule by mouth, but it shows these are active proteins.
Papain is rated possibly unsafe in pregnancy due to safety concerns with concentrated forms. Bromelain and the other enzymes lack sufficient safety data in pregnancy and lactation — talk with your doctor or pharmacist before use if you're pregnant or nursing.

Maxi-Zyme Caps by Country Life contains two ingredients with documented interactions: papain and bromelain.
The most serious concern is papain's moderate interaction with warfarin (Coumadin), a blood thinner. Papain may increase warfarin's blood-thinning effects and side effects — one patient on stable warfarin developed dangerously high bleeding risk (INR of 7.4) after taking a papain-containing supplement.

Bromelain also carries a moderate interaction with anticoagulants and antiplatelet drugs (blood thinners and aspirin-like medications). Bromelain may have blood-thinning activity of its own and could raise your bleeding risk if combined with these drugs.
There's also a minor interaction with tetracycline antibiotics; bromelain may theoretically increase their absorption, though one study in healthy adults found no practical difference.
Protease, lactase, lipase, and phytase have no documented interactions in our data. We could not check alpha-galactosidase and alpha-amylase — we hold no interaction data for them.

Bromelain
Anticoagulant/Antiplatelet Drugs
Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. However, observational research has found that topical bromelain debridement is not associated with increases or decreases in laboratory markers of coagulation when compared with surgical debridement.
Tetracycline Antibiotics
Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
Papain
Warfarin (Coumadin)
Theoretically, papain might increase the effects and side effects of warfarin.
In one case report, a patient previously stable on warfarin was found to have an international normalization ratio (INR) of 7.4, which was attributed to ingestion of a supplement containing papain from papaya extract.
