Neem by Banyan Botanicals is a single-ingredient liquid made from organic neem leaf extract. The formula also contains organic grain alcohol and deionized water as inactive ingredients to preserve and deliver the neem.
Does it work?
Moderate evidence
Neem leaf is possibly effective for gingivitis (gum inflammation) and lice. It's also possibly effective for dental plaque.
For acne and tungiasis (a parasitic skin infection), the evidence isn't strong enough to rate it either way.
How safe is it?
Well-documented data
Neem leaf preparations appear well tolerated when used short-term in adults, though thorough safety studies are limited. Neem oil and seed extracts, however, can be toxic if swallowed — especially in children, where oral neem oil has caused serious harm.
Topically, neem seems safe in both kids and adults, though it may cause contact dermatitis in sensitive skin. Rare but serious oral effects reported in adults include heart rhythm problems and kidney damage.
Neem is likely unsafe in pregnancy and should be avoided — it may affect fertility. There isn't enough safety data to recommend it while breastfeeding.

Before taking neem, talk to your doctor or pharmacist if you use antidiabetes medications (risk of low blood sugar), immunosuppressants, or any drug your liver metabolizes — including heart drugs, blood thinners, and many others. Neem may slow how your body clears these drugs, raising their levels.
These interactions are theoretical so far in humans but have been seen in lab work.

Neem by Banyan Botanicals can interact with a number of medications through its neem leaf content.
The most serious concern is with antidiabetes drugs — neem may lower your blood sugar further, raising the risk of hypoglycemia (low blood sugar). Beyond that, neem may theoretically increase the levels of several drug types by slowing how your body metabolizes them: CYP1A2 substrates, CYP2C8 substrates, CYP2C9 substrates, CYP3A4 substrates, and P-glycoprotein substrates.
So far, these interactions haven't been reported in people, but they've been seen in lab research. Your liver and kidneys use these pathways to clear many common medications, so the effect could be real.

Antidiabetes Drugs
Neem might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Some clinical research shows that neem can lower blood glucose levels in adults with type 2 diabetes, including those already taking metformin.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.
Cytochrome P450 2C8 (Cyp2C8) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C8 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP2C8 enzymes. So far, this reaction has not been reported in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C9 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP2C9 enzymes. So far, this reaction has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Immunosuppressants
Theoretically, neem might decrease the effectiveness of immunosuppressants.
Animal research suggests that neem might have immunostimulant effects.
P-Glycoprotein Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
