Acetaminophen, Chlorpheniramine, Phenylephrine, Salicylamide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo cold-and-cough product (in Rhinogesic) mixes a pain reliever and fever reducer with an antihistamine, a decongestant, and another pain reliever. The concern with neem is that, in test-tube studies only, neem leaf extract slows down a liver enzyme called CYP1A2. If that enzyme runs slower, certain medicines it processes could hang around a bit longer in your body.
Here's the honest part: this has only been seen in the lab, never reported in real people, so I wouldn't lose sleep over it. None of these ingredients is a prodrug, so the theoretical worry would be slightly stronger effects, things like drowsiness, dry mouth, or jitteriness. To be safe, run it by your pharmacist or doctor before pairing neem with this product.

In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

Here's the simple version. This cold and pain combo (Rhinogesic) contains acetaminophen, chlorpheniramine, phenylephrine, and salicylamide. In a test tube, neem leaf extract seems to slow down a liver enzyme called CYP3A4 that helps clear certain medicines. If that slowdown happened in the body, levels of the affected ingredients could creep up a bit, which might mean more drowsiness from the chlorpheniramine, or more of phenylephrine's jittery, blood-pressure-raising effects.
That said, this is only laboratory evidence and has never actually been seen in people. So I wouldn't lose sleep over it. Concentrated neem supplements are more of a question mark than tiny amounts. If you regularly take both, just give your pharmacist or doctor a quick heads-up.
