Nutrizyme contains 10 ingredients, most of them digestive enzymes. Papain and bromelain are proteases (protein-digesting enzymes) derived from papaya and pineapple.
Chymotrypsin and trypsin are also proteases, and pancreatin is a blend of multiple pancreatic enzymes. The product also includes rutin, a plant compound with antioxidant properties; shiitake mushroom powder; VegPeptase, a plant-derived protease; and soy isoflavones — plus one ingredient, Dionaea muscipula extract, we have limited data on.
The remaining ingredients are inactive fillers and binders that help form the tablet.

The evidence for most of Nutrizyme's ingredients is sparse. Chymotrypsin is rated effective for cataracts, but the evidence for the other ingredients — papain, bromelain, rutin, trypsin, shiitake, and the rest — either falls short of reliable proof or hasn't been established in the data we hold.

How safe is it?
Well-documented data
Papain is generally tolerated as a food but may irritate the digestive tract and trigger allergic reactions in sensitive people — avoid it during pregnancy due to safety concerns. Bromelain is well tolerated short-term and most commonly causes mild stomach upset like diarrhea or gas; avoid it during pregnancy and breastfeeding.
Rutin may cause headache, flushing, or rash in some people and should be avoided in pregnancy unless your doctor advises otherwise. Chymotrypsin and trypsin are generally tolerated for short-term use, but both carry a rare risk of allergic reactions, including anaphylaxis; avoid during pregnancy and breastfeeding.
Shiitake mushroom is fine as a food but concentrated supplements can cause stomach upset, and raw mushroom can cause a distinctive skin eruption called shiitake dermatitis. The most serious interaction involves papain, which may increase the effects and side effects of warfarin (Coumadin), a blood thinner — in one case, a patient's blood-thinning level spiked dangerously after taking a papain-containing supplement.

Bromelain, another enzyme in this product, may increase bleeding risk if you're on anticoagulants or antiplatelet drugs like aspirin or clopidogrel (Plavix). Bromelain may also theoretically raise tetracycline antibiotic levels, though human studies show mixed results.
Rutin, a flavonoid, might increase the blood-sugar-lowering effects of antidiabetes medications and raise your low blood sugar (hypoglycemia) risk. Shiitake mushroom powder may interfere with immunosuppressant drugs used after transplant or for autoimmune conditions, and it may theoretically decrease levels of drugs metabolized by the liver enzyme CYP2D6 — a large group that includes some antidepressants and heart medications.
Chymotrypsin and trypsin, which we checked, show no documented interactions in our data. We could not check pancreatin, soy isoflavones, or Dionaea muscipula extract — no monograph data is on file for them.

Shiitake Mushroom powder
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.
Immunosuppressants
Theoretically, taking shiitake mushroom might decrease the effects of immunosuppressive therapy.
In vitro evidence suggests that shiitake mushroom extracts stimulate immune function.
Bromelain
Anticoagulant/Antiplatelet Drugs
Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. However, observational research has found that topical bromelain debridement is not associated with increases or decreases in laboratory markers of coagulation when compared with surgical debridement.
Tetracycline Antibiotics
Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
Rutin
Antidiabetes Drugs
Theoretically, taking rutin with antidiabetes drugs might increase the risk of hypoglycemia.
Animal research suggests that rutin has hypoglycemic effects.
Papain
Warfarin (Coumadin)
Theoretically, papain might increase the effects and side effects of warfarin.
In one case report, a patient previously stable on warfarin was found to have an international normalization ratio (INR) of 7.4, which was attributed to ingestion of a supplement containing papain from papaya extract.
