Onion as a food is generally well tolerated. When taken as a concentrated supplement, though, the safety data is much thinner and less studied.

The most common side effects from oral use are abdominal pain and heartburn; the stomach distress from onion powder tends to be short-lived. Rarely, sensitive people can have serious reactions like anaphylaxis.

Onion allergy is uncommon overall, but when it does happen, symptoms can include throat tightness, severe hives, and in rare cases anaphylaxis from cooked onion. One person reported the smell alone triggered migraines.

Theoretically, concomitant use of anticoagulant or antiplatelet drugs with onion might increase the risk of bleeding.
In vitro research shows that onion inhibits platelet aggregation.

Theoretically, taking onion might increase the levels and clinical effects of drugs metabolized by CYP2E1.
Animal research shows that taking onion powder inhibits CYP2E1. However, this interaction has not been reported in humans.
