P3E is a digestive enzyme blend with 18 active ingredients designed to break down different foods. The product contains three proteases (Protease 3, 4.5, and 6) to break down proteins, lipase to digest fats, lactase to help with milk sugar (lactose), amylase and glucoamylase to handle starches, and several others—cellulase, hemicellulase, invertase, alpha-galactosidase, peptidase, phytase, bromelain, beta-glucanase, pectinase, and diastase—that work on fiber, plant compounds, and other carbohydrates.

The capsule itself is made from vegetable material, and the inactive ingredients include rice extract.

Most of these enzymes are generally well tolerated in adults, though digestive upset and allergic reactions are possible—particularly with the proteases and bromelain. Bromelain specifically can cause diarrhea, flatulence, stomach upset, headache, and rarely allergic responses (IgE-mediated), especially if inhaled.

Occupational exposure to some of these enzymes, particularly powdered protease and phytase, has rarely triggered allergic asthma and similar reactions, though that's not a typical concern with a capsule taken by mouth. For pregnancy: safety has not been established for the three proteases, lipase, phytase, and bromelain, so they're best avoided unless your doctor advises otherwise.

Lactase is likely safe in pregnancy. For breastfeeding: lactase is likely safe, but there's little data for the proteases, lipase, phytase, and bromelain—check with your doctor or pharmacist first.

Anticoagulant/Antiplatelet Drugs

Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. However, observational research has found that topical bromelain debridement is not associated with increases or decreases in laboratory markers of coagulation when compared with surgical debridement.

Tetracycline Antibiotics

Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
