Pan-Ox-5 Ingredients & Drug Interactions
What is this page for?
First and foremost: checking Pan-Ox-5 against your medications. The heart of this page is the interaction checker and the full interaction report — how this product’s ingredients may interact with prescription and over-the-counter medicines you may be taking.
Around that, we add a pharmacist’s high-level view of the product as a whole — what’s inside, the evidence for its stated use, how transparent the label is, and what safety data exists — so you can see the full picture in one place. It’s educational information from our licensed clinical databases and the clinical staff at HelloPharmacist — not medical advice — and we don’t sell or endorse products. Our editorial policy

HelloPharmacist Scorecard of Pan-Ox-5 by Douglas Laboratories
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Low disclosure
Pan-Ox-5 has 13 ingredients, 10 of which are active. The main components are digestive enzymes: amylase, protease, lipase (listed twice), pepsin, papain, diastase, and rennet — these break down carbohydrates, proteins, and fats to help digestion.
The product also contains betaine hydrochloride (which raises stomach acid), ox bile powder (which helps fat digestion), and whole-food additions like citrus pectin and beet root powder. The remaining four ingredients are inactive fillers: cellulose, vegetable stearate, rice bran, and silica.
Does it work?
Moderate evidence
The evidence for most ingredients in this product is sparse. Beet root powder shows possibly effective evidence for athletic performance and reducing exercise-induced muscle soreness, but most other active ingredients lack reliable evidence.
Citrus pectin is possibly effective for high cholesterol, but evidence for it to help with prediabetes or radiation exposure isn't established in our data. For the digestive enzymes — protease, lipase, pepsin, papain, and the others — we have no effectiveness ratings on file.
Betaine HCl, ox bile, and diastase similarly lack established evidence in our database.
How safe is it?
Well-documented data
Protease and pepsin are generally well tolerated but may cause digestive upset or allergic reactions; both should be avoided in pregnancy unless prescribed, and breastfeeding use isn't recommended without medical advice. Lipase is generally well tolerated in supplement form, though people with serious digestive conditions should use it only under medical guidance; safety data in pregnancy and breastfeeding is limited.
Citrus pectin is generally well tolerated from food and typical supplements; allergic reactions are rare but possible. Beet root powder is widely eaten and well tolerated, though concentrated supplements lack safety data — expect harmless red or black stools.
Papain should be avoided in pregnancy due to safety concerns about concentrated forms, and breastfeeding safety isn't well established. Betaine HCl may irritate your stomach and is unsafe if you have ulcers, gastritis, or true acid reflux; it should be avoided in pregnancy and breastfeeding.
We have no pregnancy or breastfeeding data on file for amylase, glutamic acid, ox bile, diastase, or rennet.
Meds to double-check
Moderate interaction found
Check with your pharmacist if you take warfarin (increased bleeding risk is the top concern here). Also screen any lovastatin, tetracycline antibiotics, or digoxin — pectin can lower their effectiveness.
If you take medications that rely on liver enzymes CYP3A4 or CYP1A2, acid-lowering drugs (antacids, H2-blockers, PPIs), or blood pressure medicines, bring your medication list in before starting.
The bottom line
Scorecard at a glanceFormula with limited ingredient disclosure with some supporting evidence for its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Pan-Ox-5 is a digestive enzyme blend aimed at people with sluggish digestion, but it's not right for everyone. If you take warfarin, lovastatin, tetracycline, digoxin, antacids, H2-blockers, PPIs, or medications broken down by liver enzymes, you need to check this with your pharmacist first.
Talk to us before you start, especially if you have ulcers or are pregnant or breastfeeding.

Pan-Ox-5 by Douglas Laboratories Drug Interactions
HelloPharmacist Interaction Report
Pan-Ox-5 by Douglas Laboratories contains several digestive enzymes and plant ingredients, and yes — it does interact with medications.
The most serious interaction involves papain, one of its proteolytic enzymes. Papain may increase the effects of warfarin (a blood thinner), raising your bleeding risk; one case report documented a dangerous spike in INR (a measure of blood thinning) after someone took a papain-containing supplement while stable on warfarin.
Read the full breakdown — every affected drug type, severity by severity
Citrus pectin in this product interacts with three drug types. It may reduce the absorption and effectiveness of lovastatin and tetracycline antibiotics (both Moderate severity), and it can significantly reduce digoxin absorption — a critical heart medication — also Moderate severity.
Beet root powder raises another concern: it may inhibit or induce liver enzymes (CYP3A4 and CYP1A2) that break down many medications, potentially raising drug levels or lowering them. One Moderate-severity interaction affects CYP3A4 substrates; a Minor one affects CYP1A2 substrates and antihypertensive drugs.
Betaine HCl works against acid-lowering medications — antacids, H2-blockers, and proton pump inhibitors (PPIs) — all Minor severity.
Protease and pepsin (both proteolytic enzymes) have been checked and carry no documented interactions. Altogether, these interactions span 892 individual medications.

Beet root powder
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, beet might increase the levels of CYP3A4 substrates.
In vitro research suggests that betanin, the major pigment in beet, competitively inhibits CYP3A4 in a dose-dependent manner similarly to strong CYP3A4 inhibitor ketoconazole.
Antihypertensive Drugs
Beet and beetroot contain nitrates, which can cause vasodilation, potentially leading to lower blood pressure. However, a study published in the European Journal of Clinical Nutrition using concentrated beetroot juice found no significant impact on blood pressure or heart rate in different age groups. Other small clinical studies suggest that while beet consumption might transiently lower blood pressure due to vessel dilation, there's no consistent evidence of a lasting effect. Overall, the theoretical risk of reduced blood pressure due to beet's nitrate content exists, but studies generally indicate a low and temporary impact rather than a sustained decrease.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, beet might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research suggests that beet induces CYP1A2 enzymes.
Betaine HCl
Antacids
Betaine hydrochloride increases stomach acidity and could decrease the effects of antacids.
In human research, betaine hydrochloride increases stomach acidity. Antacids are taken to decrease stomach acidity. Theoretically, taking betaine hydrochloride along with antacids might decrease the effects of the antacids.
H2-Blockers
Betaine hydrochloride increases stomach acidity and could decrease the effects of H2-blockers.
In human research, betaine hydrochloride increases stomach acidity. H2-blockers are used to decrease stomach acidity. Theoretically, taking betaine hydrochloride along with H2-blockers might decrease the effects of H2-blockers.
Proton Pump Inhibitors (Ppis)
Betaine hydrochloride increases stomach acidity and could decrease the effects of PPIs.
In human research, betaine hydrochloride increases stomach acidity. PPIs are used to decrease stomach acidity. Theoretically, taking betaine hydrochloride along with PPIs might decrease the effects of PPIs
Citrus Pectin
Digoxin (Lanoxin)
Theoretically, pectin might reduce the absorption of digoxin, potentially decreasing its effectiveness.
A small clinical study shows that taking digoxin with a kaolin-pectin suspension reduces the absorption of digoxin by about 62%. It is unclear if these effects are due to pectin, kaolin, or the combination.
Lovastatin (Mevacor)
Theoretically, pectin might reduce the absorption of lovastatin, potentially decreasing its effectiveness.
Case reports suggest that concomitant use of pectin and lovastatin might reduce the cholesterol-lowering effect of lovastatin, possibly due to reduced intestinal absorption of lovastatin.
Tetracycline Antibiotics
Theoretically, pectin might reduce the absorption of tetracycline antibiotics, potentially decreasing their effectiveness.
A small clinical study shows that taking tetracycline with bismuth subsalicylate in a kaolin-pectin suspension reduces the absorption of tetracycline by about 34%. It is unclear if these effects are due to pectin, kaolin, bismuth subsalicylate, or the combination.
Papain
Warfarin (Coumadin)
Theoretically, papain might increase the effects and side effects of warfarin.
In one case report, a patient previously stable on warfarin was found to have an international normalization ratio (INR) of 7.4, which was attributed to ingestion of a supplement containing papain from papaya extract.
