Squalene is generally tolerated in the short term, but human safety data are limited and product quality varies across brands. One case of severe liver inflammation (hepatitis) was reported with shark liver oil taken twice daily for 2 weeks; the patient had abdominal discomfort, yellowing of the skin and eyes (jaundice), weakness, itching, and elevated liver enzyme levels.

High doses — 15 grams daily for 4 weeks — raised total cholesterol by about 41.5 mg/dL and lowered HDL cholesterol, though these changes reversed after stopping. Lower doses (1.5 grams daily for 8 weeks) showed no significant effect on cholesterol.

Rare cases of a type of lung inflammation (lipoid pneumonia) from accidental inhalation have also been linked to squalene and shark liver oil. The safety data advise against use during pregnancy and breastfeeding.
