Tansy carries significant safety warnings. It contains thujone, a toxic compound, and internal use is generally discouraged because of poisoning risk — thujone toxicity can cause rapid pulse, irregular heartbeat, trouble breathing, severe stomach upset, vomiting, and in rare cases, serious liver or kidney damage or death.

Tansy has a traditional history of being used to bring on menstruation or cause abortion, so it must not be used in pregnancy. Safety is not established while breastfeeding, and the thujone content makes it unsafe during lactation.

Topical contact with tansy may cause skin reactions like eczema or photosensitivity (sun-triggered rashes) in some people.
