Testosterone Premium Ingredients & Drug Interactions
What is this page for?
First and foremost: checking Testosterone Premium against your medications. The heart of this page is the interaction checker and the full interaction report — how this product’s ingredients may interact with prescription and over-the-counter medicines you may be taking.
Around that, we add a pharmacist’s high-level view of the product as a whole — what’s inside, the evidence for its stated use, how transparent the label is, and what safety data exists — so you can see the full picture in one place. It’s educational information from our licensed clinical databases and the clinical staff at HelloPharmacist — not medical advice — and we don’t sell or endorse products. Our editorial policy

HelloPharmacist Scorecard of Testosterone Premium by Premier Research Labs
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Low disclosure
Testosterone Premium contains 7 active ingredients. Maca, American ginseng, cordyceps mycelia extract, pumpkin powder, saw palmetto, LJ100 (an extract of Eurycoma longifolia), and Testofen (fenugreek) are all intended to support testosterone levels and male sexual function.
The product also contains two inactive ingredients—cellulose and water—which serve as fillers and capsule material.
Does it work?
Moderate evidence
The evidence for testosterone support is mixed. Testofen (fenugreek) is possibly effective for sexual dysfunction and sexual arousal, and possibly effective for diabetes.
LJ100 (Eurycoma longifolia) is possibly effective for sexual desire. Pumpkin powder is possibly effective for benign prostate enlargement (BPH).
However, cordyceps is rated possibly ineffective for athletic performance, and saw palmetto is rated possibly ineffective for BPH. The remaining ingredients—maca, American ginseng, and cordyceps for most of their uses—lack sufficient evidence to rate.
If your goal is sexual function or prostate health, fenugreek and pumpkin have the most support; if you're chasing athletic gains, the evidence doesn't back cordyceps for that purpose.
How safe is it?
Well-documented data
Most ingredients are generally well tolerated short-term, though long-term safety data is limited for several. Common mild side effects include abdominal discomfort, diarrhea, nausea, headache, and dizziness.
Fenugreek and cordyceps may cause gastrointestinal upset. Saw palmetto rarely causes a skin rash or heart rhythm changes.
Cordyceps carries a rare case report of liver inflammation (hepatitis) tied to a supplement containing it, though it's unclear whether cordyceps itself was the cause or a contaminant. If you have testosterone-related concerns (acne, insulin resistance, liver problems), Eurycoma longifolia may pose additional risk if it raises your testosterone beyond normal.
Maca from fresh, uncooked plant material may cause stomach pain. Avoid this product during pregnancy: most ingredients are rated unsafe or lack sufficient safety data, and saw palmetto has possible hormonal effects.
During breastfeeding, data is insufficient for most ingredients; fenugreek is sometimes used to boost milk supply but evidence is limited—talk to your provider first.
Meds to double-check
Major interaction found
Double-check blood thinners like warfarin, clopidogrel, and aspirin—American ginseng and fenugreek may interfere, and cordyceps and saw palmetto may increase bleeding risk. Diabetes medications are a concern: American ginseng and fenugreek can lower blood sugar further.
Birth control (estrogen-based) and estrogen therapy may be less effective with saw palmetto. Blood pressure drugs, especially propranolol and metoprolol, may be affected by LJ100 and fenugreek.
Seizure medications like phenytoin and asthma drugs like theophylline may be less effective with fenugreek. Monoamine oxidase inhibitors (MAOIs, older antidepressants) and lithium require caution with American ginseng and pumpkin respectively.
Sildenafil (Viagra) levels may drop with fenugreek.
The bottom line
Scorecard at a glanceFormula with limited ingredient disclosure with some supporting evidence for its stated purpose. Major medication interactions have been identified, and safety information is well characterized.
This product is aimed at men seeking testosterone support and sexual function improvement, with the strongest evidence for fenugreek and pumpkin. If you take blood thinners, diabetes medications, birth control, any antidepressants, or heart drugs like propranolol or metoprolol, you need to check your exact medications with the tool on this page before starting—the interaction risks are real.
Pregnant women and those breastfeeding should talk to their provider. If you have heart, liver, or hormonal concerns, mention this product to your pharmacist before you use it.

Testosterone Premium by Premier Research Labs Drug Interactions
HelloPharmacist Interaction Report
Testosterone Premium by Premier Research Labs contains several ingredients with documented interactions affecting a broad range of medications.
American ginseng carries the most serious interaction: it significantly reduces the effectiveness of warfarin (Coumadin), a blood thinner, by lowering your clotting time (INR), which could raise your blood clot risk—this is a Major severity concern.
Read the full breakdown — every affected drug type, severity by severity
Several other medication types carry Moderate interactions. Cordyceps, pumpkin powder, and saw palmetto may increase bleeding risk if you're on blood thinners or antiplatelet drugs like aspirin or clopidogrel.
American ginseng and fenugreek (Testofen) may lower blood sugar too much if you take diabetes medications, raising the risk of low blood sugar (hypoglycemia). Saw palmetto and fenugreek may reduce the effectiveness of estrogen-based contraceptives.
Fenugreek may also reduce how well your body absorbs theophylline (an asthma medication), phenytoin (Dilantin, a seizure drug), or sildenafil (Viagra). American ginseng and fenugreek may interfere with certain antidepressants called monoamine oxidase inhibitors (MAOIs).
Pumpkin powder may increase lithium levels if you take it for bipolar disorder. Eurycoma longifolia (LJ100) may reduce propranolol (Inderal) blood pressure control and theoretically increase levels of drugs cleared by liver enzymes (CYP1A2, CYP2C19, and CYP2A6).
Saw palmetto and cordyceps may also have Minor interactions with testosterone itself—additive effects that could amplify testosterone-related side effects. We could not check one ingredient in the blend; Maca has no interactions documented in our data, though that doesn't guarantee none exist elsewhere.
Altogether, these interactions span 727 individual medications. Use the medication checker below to verify your exact drugs before you start.

For each ingredient in Testosterone Premium with known interactions, here are the types of medications they can affect. Open any type for the detail — or search your exact drug in the checker above.
Testofen
Anticoagulant/Antiplatelet Drugs
Theoretically, fenugreek might have additive effects when used with anticoagulant or antiplatelet drugs.
Some of the constituents in fenugreek have antiplatelet effects in animal and in vitro research. However, common fenugreek products might not contain sufficient concentrations of these constituents for clinical effects. A clinical study in patients with coronary artery disease or diabetes shows that taking fenugreek seed powder 2.5 grams twice daily for 3 months does not affect platelet aggregation, fibrinolytic activity, or fibrinogen levels .
Antidiabetes Drugs
Theoretically, fenugreek seed might have additive hypoglycemic effects when used with antidiabetes drugs.
Clinical research shows that fenugreek seed can reduce fasting blood glucose and 2-hour postprandial glucose levels in adults with type 2 diabetes.
Clopidogrel (Plavix)
Theoretically, fenugreek seed might alter the clinical effects of clopidogrel by inhibiting its conversion to the active form.
Animal research shows that fenugreek seed 200 mg/kg daily for 14 days increases the maximum serum concentration of clopidogrel by 21%. It is unclear how this affects the pharmacokinetics of the active metabolite of clopidogrel; however, this study found that concomitant use of fenugreek seed and clopidogrel prolonged bleeding time by an additional 11%.
Metoprolol (Toprol)
Theoretically, fenugreek seed might have additive hypotensive effects when used with metoprolol.
Animal research shows that fenugreek seed 300 mg/kg daily for 2 weeks decreases systolic and diastolic blood pressure by 9% and 11%, respectively, when administered alone, and by 15% and 22%, respectively, when given with metoprolol 10 mg/kg.
Phenytoin (Dilantin)
Theoretically, fenugreek might decrease plasma levels of phenytoin.
Animal research shows that taking fenugreek seeds for 1 week decreases maximum concentrations and the area under the curve of a single dose of phenytoin by 44% and 72%, respectively. This seems to be related to increased clearance. So far, this interaction has not been reported in humans.
Sildenafil (Viagra)
Theoretically, concurrent use of sildenafil and fenugreek might reduce levels and therapeutic effects of sildenafil.
Animal research shows that taking fenugreek seeds for 1 week reduces maximum concentrations and the area under the curve of a single dose of sildenafil by 27% and 48%, respectively. So far, this interaction has not been reported in humans.
Theophylline
Theoretically, fenugreek may reduce the levels and clinical effects of theophylline.
Animal research shows that fenugreek 50 grams daily for 7 days reduces the maximum serum concentration (Cmax) of theophylline by 28% and the area under the plasma drug concentration-time curve (AUC) by 22%.
Warfarin (Coumadin)
Theoretically, fenugreek might have additive effects with warfarin and increase the international normalized ratio (INR).
Some fenugreek constituents have antiplatelet effects, although these might not be present in concentrations that are clinically significant. In one case report, a patient taking warfarin experienced an increased INR when starting to take fenugreek in combination with boldo.
Antihypertensive Drugs
Fenugreek may also have an additive effect on blood pressure-lowering medications. Studies on animals have shown that fenugreek seed can decrease both systolic and diastolic blood pressure by up to 22% when combined with metoprolol. Therefore, it is essential to monitor your blood pressure regularly if you are taking fenugreek and metoprolol together or any other antihypertensive drugs.
Cordyceps Mycelia Extract
Anticoagulant/Antiplatelet Drugs
Theoretically, cordyceps may increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
In vitro and animal research suggests that cordyceps extract inhibits platelet aggregation and function. However, this interaction has not been reported in humans.
Immunosuppressants
Theoretically, concurrent use of cordyceps might interfere with immunosuppressive therapy.
Animal and in vitro research suggests that cordyceps stimulates the immune system. However, limited clinical research suggests that taking cordyceps may lower the necessary therapeutic dose of the immunosuppressant cyclosporine, which suggests that cordyceps may have an immunosuppressive effect.
Testosterone
Theoretically, concurrent use of cordyceps and testosterone might have additive effects.
Animal research suggests that cordyceps can increase testosterone levels. The clinical significance of this finding is unclear.
LJ100
Propranolol (Inderal)
Eurycoma longifolia can reduce the levels and clinical effects of propranolol.
A small clinical study in healthy persons shows that taking a single dose of a water-based Eurycoma longifolia extract 200 mg, in combination with a single dose of propranolol 80 mg, reduces the propranolol area under the curve (AUC) by 29%, reduces the peak concentration by 42%, and increases time to peak concentration by 86% when compared with control. Since the elimination half-life of propranolol did not change, it seems that Eurycoma longifolia alters the kinetics of propranolol by decreasing its absorption in the gut, and not by altering its metabolism. It is not known if separating administration will prevent this interaction.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, Eurycoma longifolia might increase levels CYP1A2 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP1A2 enzymes. This effect has not been reported in humans.
Cytochrome P450 2A6 (Cyp2A6) Substrates
Theoretically, Eurycoma longifolia might increase levels of CYP2A6 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2A6 enzymes. This effect has not been reported in humans.
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, Eurycoma longifolia might increase levels of CYP2C19 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2C19 enzymes. This effect has not been reported in humans.
Testosterone
Theoretically, Eurycoma longifolia may further increase levels of testosterone.
A clinical study in aging males with testosterone levels below 300 ng/dL shows that taking a specific water extract of Eurycoma longifolia roots (Physta; Biotropics Malaysia) 100-200 mg daily with breakfast for 12 weeks increases total testosterone levels by 8% to 11% when compared with placebo. It is unclear whether this increase would occur in individuals with normal testosterone levels.
American Ginseng
Warfarin (Coumadin)
American ginseng seems to decrease the effectiveness of warfarin therapy.
Healthy patients receiving warfarin 5 mg daily, who also take American ginseng 1 gram twice daily, seem to have a significantly reduced international normalized ratio (INR).
Antidiabetes Drugs
Theoretically, taking American ginseng with antidiabetes drugs might increase the risk of hypoglycemia.
American ginseng seems to lower postprandial blood glucose. Theoretically, concomitant use with antidiabetes drugs might enhance blood glucose lowering effects and possibly cause hypoglycemia.
Immunosuppressants
Theoretically, American ginseng use might interfere with immunosuppressive therapy.
American ginseng seems to stimulate immune function. Theoretically, American ginseng might decrease the effectiveness of immunosuppressant drugs.
Monoamine Oxidase Inhibitors (Maois)
Theoretically, American ginseng can interfere with MAOI therapy.
There is one case report of insomnia, headache, and tremors when an unspecified ginseng product was used with phenelzine (Nardil), an MAOI. There is also one case report of hypomania when an unspecified ginseng product was used with phenelzine. Theoretically, American ginseng may interfere with MAOI therapy.
Saw Palmetto
Anticoagulant/Antiplatelet Drugs
Saw palmetto might increase the risk of bleeding with anticoagulant or antiplatelet drugs.
Saw palmetto is reported to prolong bleeding time. Theoretically, it might increase the risk of bleeding when used concomitantly with anticoagulant or antiplatelet drugs.
Contraceptive Drugs
Saw palmetto might reduce the effectiveness of contraceptive drugs.
Saw palmetto might have antiestrogenic effects. Theoretically, it might interfere with contraceptive drugs taken concomitantly.
Estrogens
Saw palmetto might reduce the effectiveness of estrogens.
Saw palmetto might have antiestrogenic effects. Theoretically, it might interfere with estrogens taken concomitantly.
Pumpkin, Powder
Lithium
Pumpkin might reduce excretion and increase levels of lithium.
Pumpkin is thought to have diuretic properties. Theoretically, this might reduce excretion and increase levels of lithium. The dose of lithium might need to be decreased.
