The most serious concern is a moderate-severity interaction with blood thinners (anticoagulants and antiplatelet drugs): resveratrol, the active compound in hu zhang, appears to thin the blood itself, which could raise your bleeding risk if you're already on medications like warfarin or aspirin.

Beyond that, hu zhang may slow how your liver breaks down several categories of drugs. In test tubes, resveratrol inhibits multiple liver enzymes (CYP1A2, CYP2C19, CYP3A4, and CYP2E1), which theoretically could raise levels of medications processed by these pathways — though this hasn't been confirmed in people yet.
One small human study of resveratrol at a much higher dose (3,000 mg daily) didn't require dose changes to medications, but at this product's lower dose we don't have that data. There's also a theoretical concern that hu zhang may compete with or reduce the effects of estrogen replacement therapy, and a minor animal study suggests it might intensify the effects of carbamazepine (Tegretol), a seizure medication.
Although trans-resveratrol is listed as an ingredient, the product notes that its component ingredients are listed individually — so the resveratrol effects come through the polygonum cuspidatum extract listed here. Altogether, these interactions span 827 individual medications.

Trans-Resveratrol
Anticoagulant/Antiplatelet Drugs
Resveratrol may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Resveratrol seems to have antiplatelet effects.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, resveratrol might increase levels of drugs metabolized by CYP1A2.
In vitro research shows that resveratrol can inhibit CYP1A2 enzymes. However, this interaction has not been reported in humans.
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, resveratrol might increase levels of drugs metabolized by CYP2C19.
In vitro research shows that resveratrol can inhibit CYP2C19 enzymes. However, this interaction has not been reported in humans.
Cytochrome P450 2E1 (Cyp2E1) Substrates
Resveratrol might increase levels of drugs metabolized by CYP2E1.
In vitro research suggests that resveratrol inhibits CYP2E1 isoenzyme. Also, a pharmacokinetic study shows that taking resveratrol 500 mg daily for 10 days prior to taking a single dose of chlorzoxazone 250 mg increases the maximum concentration of chlorzoxazone by about 54%, the area under the curve of chlorzoxazone by about 72%, and the half-life of chlorzoxazone by about 35%. Chlorzoxazone is used as a probe drug for CYP2E1.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, resveratrol might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that resveratrol can inhibit the CYP3A4 enzyme. However, clinical research shows that taking resveratrol 3000 mg daily for 8 weeks does not necessitate dose adjustments to medications metabolized by CYP3A4.
