Acetaminophen, Dextromethorphan falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Hu Zhang interacts with.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP1A2 enzyme. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo. The resveratrol in Hu Zhang has been shown, in test-tube studies only, to slow down a liver enzyme called CYP1A2. In theory, slowing that enzyme could let certain medicines linger a bit longer in your system. For your Tylenol Cough product, the dextromethorphan piece is the one mostly tied to this pathway, so the idea is you might feel its effects more strongly, things like drowsiness, dizziness, or an upset stomach.
I want to be honest though: this is a theoretical concern based on lab work, and it hasn't actually been reported in people. The acetaminophen side isn't the real issue here. If you use Hu Zhang regularly, just mention it to your pharmacist or doctor so they can keep an eye on things.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.

Here's the gist with Tylenol Cough (acetaminophen and dextromethorphan) and Hu Zhang. This herb is rich in resveratrol, and in test-tube studies resveratrol can mildly slow down CYP3A4, one of the liver enzymes that helps clear the dextromethorphan part of this combo. If that happened in real life, dextromethorphan levels could nudge upward, which might mean more drowsiness, dizziness, or a foggy, lightheaded feeling.
That said, this is only a theoretical concern based on lab work. An actual human study using a hefty 3000 mg of resveratrol daily didn't force any dose changes for CYP3A4 medications, so the real-world risk looks small. Still, if you plan to take both regularly, run it by your pharmacist or doctor first.

Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP2E1 enzyme. Also, a pharmacokinetic study shows that taking resveratrol 500 mg daily for 10 days prior to taking a single dose of chlorzoxazone 250 mg increases the maximum concentration of chlorzoxazone by about 54%, the area under the curve of chlorzoxazone by about 72%, and the half-life of chlorzoxazone by about 35%. Chlorzoxazone is used as a probe drug for CYP2E1.

Here's the connection worth knowing. The acetaminophen in your Tylenol Cough product is partly handled by a liver enzyme called CYP2E1. Hu Zhang contains resveratrol, which in lab studies appears to slow that same enzyme down a bit. In theory, that could mean a little more acetaminophen lingering in your system.
For everyday use this is a fairly soft, theory-based concern, and the dextromethorphan part isn't really involved here. Still, acetaminophen is rough on the liver in high amounts, so I wouldn't want to stack extra strain on it. Watch your total daily acetaminophen from all sources, skip alcohol, and check with your pharmacist before pairing concentrated Hu Zhang supplements with this medicine.
