Why this applies to Colchicine

Colchicine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hu Zhang interacts with.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP3A4.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.

Here's the gist: your body clears colchicine using an enzyme called CYP3A4. Hu Zhang contains resveratrol, which in lab studies may slow that enzyme down a bit. If that happened in your body, colchicine could clear more slowly and build up, which can mean more side effects like nausea, diarrhea, stomach cramps, and (less often) muscle weakness.

I want to keep this in perspective. This concern is theoretical, based mostly on test-tube work, and a human study using a hefty resveratrol dose didn't force any medication changes. Still, colchicine has a fairly narrow safe range, so I'd run any Hu Zhang supplement past your pharmacist or doctor first rather than starting it on your own.

About Colchicine

What Is Colchicine Used For?

Brand names: Colchicine, Colsalide Improved, Mitigare, Colcrys, Gloperba, Lodoco

Colchicine tablets and capsules are used to prevent gout flares in adults, and colchicine tablets are also used to treat acute gout flares when taken at the first sign of a flare. Colchicine tablets are additionally approved for familial Mediterranean fever (FMF) — a hereditary inflammatory condition — in adults and children 4 years and older.

Lodoco (colchicine tablet) is specifically indicated to reduce the risk of heart attack, stroke, coronary revascularization, and cardiovascular death in adults with established atherosclerotic disease or multiple cardiovascular risk factors. Colchicine is not a pain reliever and should not be used for pain from other causes.
