Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hu Zhang interacts with.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP3A4.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.

Here's the idea behind this pairing: Hu Zhang contains resveratrol, and in test-tube studies resveratrol can block the CYP3A4 enzyme, one of the body's drug-processing pathways. Since Rifampin is handled in part by that pathway, the theory is that Hu Zhang could slow its breakdown a little and raise levels in your body. If that happened, you might notice more of Rifampin's usual effects, like stomach upset or, rarely, liver irritation.

I want to be honest about how weak this is. It's based on lab work, not real patients, and one human study using a big resveratrol dose didn't require any medication changes. Rifampin is actually a powerful enzyme inducer itself, so a small herbal effect is unlikely to upend your treatment. Still, because TB therapy matters so much, check with your pharmacist or doctor before combining them.

About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.

Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
