Testosterone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hu Zhang interacts with.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP3A4.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.

Here's the basic idea: your body uses an enzyme called CYP3A4 to help break down testosterone. The resveratrol in Hu Zhang might slow that enzyme down a little, at least in lab tests. If that happened in real life, testosterone could clear a bit more slowly and build up, which in theory might nudge up side effects like acne, mood swings, fluid retention, or changes in your blood counts.

I want to be honest about how shaky this is, though. This is only a theoretical concern based on test-tube data. In one human study, even very high resveratrol doses didn't force any dose changes for CYP3A4 medications. So I wouldn't lose sleep over it, but since your doctor already monitors your testosterone levels, just mention the supplement so they can keep an eye on things.

About Testosterone

What Is Testosterone Used For?

Brand names: Intrinsa, Testim, Androgel, Andropatch, Testogel, Nebido, Delatestryl, Natesto, Vogelxo

Testosterone replacement therapy is used in adult males whose bodies do not produce enough of their own testosterone. This includes primary hypogonadism — where the testes themselves fail due to causes such as Klinefelter's syndrome, injury, chemotherapy, or surgical removal — and hypogonadotropic hypogonadism, where the brain's signaling system (the hypothalamus or pituitary gland) fails to trigger testosterone production.

Testopel (subcutaneous pellets) may also be used to stimulate puberty in carefully selected males with clearly delayed puberty. The safety and efficacy of testosterone for age-related (late-onset) hypogonadism have not been established for most products, including AndroGel, Testim, Jatenzo, Kyzatrex, Natesto, Depo-Testosterone, Testosterone Cypionate, and Xyosted.
