Hu Zhang and Bendamustine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Why this applies to Bendamustine

Bendamustine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Hu Zhang interacts with.

Theoretically, hu zhang might increase levels of drugs metabolized by CYP1A2.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP1A2 enzyme. This interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the idea behind this pairing. Bendamustine is partly broken down by a liver enzyme called CYP1A2. Hu Zhang is rich in resveratrol, and in test-tube studies resveratrol can slow that same enzyme down. If it slowed it in real life, your body might clear bendamustine a little less efficiently, leaving more of it around. With a chemotherapy drug, that could mean stronger side effects like low blood counts, nausea, fatigue, fever, or a higher infection risk.

I want to be honest about the strength of this, though. It's theoretical, based on lab work, and hasn't actually been seen in people. Still, since bendamustine is potent and tightly dosed, please clear any Hu Zhang or resveratrol supplement with your oncology team first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Bendamustine page

“Bendamustine (Treanda, Belrapzo, Vivimusta) is IV chemotherapy for chronic lymphocytic leukemia and certain lymphomas, and the liver clears it through an enzyme called CYP1A2. That enzyme is why nearly all 87 supplements on this page appear here, since about 80 of them can push CYP1A2 up or down, which could either raise side effects o…”

About Hu Zhang

What is Hu Zhang?

  • Source of resveratrol
  • Heart and circulation support
  • Inflammation
  • Liver and digestive complaints (traditional)
  • Cough and respiratory symptoms (traditional)
  • Skin and joint conditions (traditional topical use)

Hu Zhang (Japanese knotweed root) is a traditional Chinese herb that is one of the richest natural sources of resveratrol and emodin. Some lab and early human research looks interesting for inflammation and heart health, but high-quality evidence in people is limited. Talk to your pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding.
