Tribulus is often well tolerated in the short term for healthy adults, though long-term safety data are limited and quality can vary between products. Serious side effects are rare but have been reported, including liver and kidney injury, seizures, and in one case a patient drinking large amounts of tribulus water daily developed severe weakness, seizures, and kidney problems that improved after stopping it and undergoing dialysis.

More common side effects — abdominal pain, cramping, nausea, vomiting, diarrhea, and constipation — occurred at similar rates to placebo in one study. The safety data advises against using this product during pregnancy because it may affect hormones and safety is not established.

Taking tribulus with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that Tribulus can lower blood glucose levels in adults with type 2 diabetes who are taking antidiabetes medications.

Animal research shows that tribulus can lower blood pressure by inhibiting angiotensin-converting enzyme (ACE). Tribulus has also demonstrated hypotensive effects in pre-hypertensive adults.
