Both diindolylmethane and indole-3-carbinol are generally well tolerated in the short term, though long-term safety hasn't been thoroughly studied. Common side effects include diarrhea, nausea, gas, headache, and rash.

Indole-3-carbinol can cause unsteadiness and imbalance, especially at higher doses — one case saw these symptoms resolve when the dose was cut in half. A rare but serious reaction called DRESS (drug rash with eosinophilia and systemic symptoms) has been reported with diindolylmethane.

For pregnancy, diindolylmethane is rated likely safe, but indole-3-carbinol has no safety data during pregnancy — talk with your doctor or pharmacist for personalized advice. Both should be avoided while breastfeeding due to insufficient safety information.
