Acetaminophen, Ibuprofen falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Indole-3-carbinol interacts with.
Animal research shows that indole-3-carbinol induces CYP1A2 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Combogesic (the acetaminophen plus ibuprofen combo) and indole-3-carbinol. This supplement may gently nudge a liver enzyme called CYP1A2 to work faster. In theory, that could speed up how your body clears these pain relievers, which might make them a little less effective for headaches, aches, or fever. So the concern here is weaker relief, not a buildup of side effects.
Honestly, this is a very weak signal. It comes only from lab and animal studies, not from real people, and everyday broccoli or cabbage almost certainly won't do anything. Concentrated I3C capsules are the bigger question mark. If you take both regularly, just mention it to your pharmacist or doctor before changing anything.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing with Combogesic (which combines acetaminophen and ibuprofen): the ibuprofen part already nudges your platelets, the cells that help blood clot, to be a little less sticky. Indole-3-carbinol may do something similar. In lab dish studies, it appears to slow platelets from clumping together. Put the two side by side and, in theory, you could bleed or bruise a bit more easily than you would on either one alone.
I want to be honest, though: this concern comes only from test-tube research, not from real people, so it's more of a caution flag than a proven problem. Eating broccoli or cabbage isn't the issue here, concentrated I3C supplements are. If you notice easy bruising, nosebleeds, or dark stools, or you take a blood thinner too, check in with your pharmacist or doctor first.
