Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
Why this applies to Bendamustine Hydrochloride
Bendamustine Hydrochloride falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Indole-3-carbinol interacts with.
Animal research shows that indole-3-carbinol induces CYP1A2 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the idea behind this pairing. Your liver uses an enzyme called CYP1A2 to help process bendamustine (Bendeka), and concentrated indole-3-carbinol supplements may speed that enzyme up. In theory, a faster enzyme could clear the drug a little quicker and nudge its blood levels down, which could mean your chemotherapy works less well than your oncology team is counting on.
I want to be straight with you though: this concern comes only from lab and animal studies, not from people, so it's very theoretical. The everyday broccoli and cabbage on your dinner plate aren't the worry here, concentrated capsules are. Since bendamustine is a serious cancer treatment, please don't add any I3C supplement without first running it by your oncologist or pharmacist.

Indole-3-carbinol (I3C) is a compound formed when you eat cruciferous vegetables like broccoli and cabbage, and it is sold as a supplement mainly for hormone-related and cell-protective effects. Most human evidence is preliminary or comes from lab and animal studies, so it should not be relied on to prevent or treat any disease. Talk to your pharmacist or doctor before using it, and avoid it in pregnancy and breastfeeding.
About Bendamustine Injection
What Is Bendamustine Injection Used For?
Brand name: Bendeka
Bendamustine hydrochloride injection is used to treat adults with chronic lymphocytic leukemia (CLL). It is also used for adults with indolent B-cell non-Hodgkin lymphoma (NHL) — a slow-growing type of blood cancer — when the disease has continued to progress during or within six months of treatment with rituximab or a rituximab-containing regimen.
For CLL, how well bendamustine works compared to other first-line treatments (aside from chlorambucil) has not been fully established.
