Propafenone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Indole-3-carbinol interacts with.

Theoretically, indole-3-carbinol might increase the metabolism of CYP1A2 substrates and lower serum concentrations.

Here's the gist for propafenone (Rythmol): this heart rhythm drug is partly handled by a liver enzyme called CYP1A2. Indole-3-carbinol, a compound from broccoli and other cruciferous veggies, may speed up that enzyme. If it does, your body could clear propafenone a little faster, which means lower levels and possibly less control of your irregular heartbeat. Propafenone is an antiarrhythmic, so a drop in its effect is something to take seriously.

The good news is this is very weak evidence, coming only from lab and animal studies, not from people. Eating normal amounts of broccoli or cabbage almost certainly won't matter. Concentrated I3C supplements are the real question, so check with your pharmacist or doctor before starting one.

About Propafenone

What Is Propafenone Used For?

Brand names: Rythmol, Arythmol

Propafenone is used to help control abnormal heart rhythms. It is prescribed to delay the return of paroxysmal atrial fibrillation or atrial flutter (episodic irregular heartbeats in the upper chambers) and paroxysmal supraventricular tachycardia (PSVT, sudden fast heartbeats starting above the heart's lower chambers) — but only in people who do not have structural heart disease.

It is also used to treat life-threatening ventricular arrhythmias (dangerous fast rhythms starting in the heart's lower chambers). Because of serious safety concerns, it is not recommended for minor or non-life-threatening heart rhythm problems.
