Most of these enzymes are generally well tolerated at normal doses. The main concern is allergic reactions: papain and bromelain can trigger itchy eyes, runny nose, sneezing, abdominal cramps, or diarrhea in people sensitive to them.

High doses of papain may cause serious stomach injury (esophageal perforation and severe gastritis), though this is rare. Bromelain commonly causes mild digestive upset — diarrhea, gas, and nausea — especially at higher doses.

Protease and phytase may also trigger allergic reactions in sensitive people. For pregnancy, the safety data advises against papain and bromelain.

Lactase is likely safe in pregnancy. For protease, lipase, and phytase, there isn't enough data — talk with your doctor before regular use.
