Metformin and sulfonylureas can modestly decrease endogenous levels of coenzyme Q10, but coenzyme Q10 supplements are not likely to be needed for patients taking these drugs.

Some antidiabetes drugs, including metformin and sulfonylureas, appear to inhibit coenzyme Q10 enzyme activity and decrease endogenous levels of coenzyme Q10. However, there is no strong evidence that coenzyme Q10 depletion is the cause of antidiabetes drug-related side effects. Furthermore, taking coenzyme Q10 does not seem to improve fasting glucose levels or glycated hemoglobin in patients with diabetes.

Coenzyme Q10 is a natural compound your cells use to produce energy, and some antidiabetes medications have been linked to modest dips in its levels. The connection with insulin specifically is not well established, and the overall evidence here is rated insignificant, meaning most people using insulin do not need to worry about this or take a supplement for it. Research so far also has not shown that adding coenzyme Q10 helps blood sugar control in people with diabetes. That said, if you have questions about your personal situation, your pharmacist or doctor is a great place to start.

Insulin can enhance the renal excretion of magnesium. Theoretically, it might lower magnesium stores; however, lower magnesium stores are common in patients with diabetes due to decreased intake and osmotic diuresis resulting in increased magnesium excretion. Magnesium helps your muscles contract, keeps your heart rhythm steady, and supports hundreds of chemical reactions throughout the body. Insulin appears to prompt the kidneys to release a little more magnesium into the urine than usual, which could theoretically nudge levels downward over time. That said, the evidence rates this as insignificant, meaning most people on insulin do not need a magnesium supplement because of the medication itself. If you have concerns, especially since low magnesium is already more common in people with diabetes for other reasons, it is worth bringing up with your pharmacist or doctor.
