Use caution when using cycloSPORINE with other drugs which are known to impair renal function like amphotericin B. During the concomitant use of a drug that may exhibit additive or synergistic renal impairment with cycloSPORINE, close monitoring of renal function (serum creatinine) should be performed. If a significant impairment of renal function occurs, reduce the dosage of the cycloSPORINE and/or coadministered drug or consider an alternative treatment12. In bone marrow transplant patients, a high incidence of nephrotoxicity was noted in those patients receiving concurrent cycloSPORINE and amphotericin B. Serum creatinine doubled in 80% of patients on concurrent medications within 5 days vs 38% of patients on cycloSPORINE alone 3. In another study also involving bone marrow transplant recipients, significant renal toxicity was observed in 83% of subjects who received amphotericin B and cycloSPORINE concurrently compared with 10% of patients who received cycloSPORINE monotherapy 4.
