Decreased cycloSPORINE effectiveness

Interaction Deep Dive

Chronic administration of armodafinil (R-enantiomer of modafinil) may cause moderate induction of CYP3A4/5 isozyme activity. Concurrent administration with a CYP3A4 substrate, such as cycloSPORINE, may result in reduced blood levels and effectiveness of cycloSPORINE1. Although not reported with armodafinil, a case report described an organ transplant recipient who experienced a 50% reduction in her cycloSPORINE blood levels following one month of therapy with modafinil 200 mg daily 2. Therefore, caution is advise if armodafinil and cycloSPORINE are used concurrently. Levels of cycloSPORINE should be monitored closely and doses adjusted accordingly 1.
