No brand names on recordHow we grade severity & evidence
Severity levels
- Contraindicated: These should generally not be used together.
- Major: Potentially serious — often needs a change or close monitoring.
- Moderate: Can be significant — usually manageable with monitoring.
- Minor: Usually limited clinical impact.
Evidence grades
- Established: Well documented — supported by controlled studies or strong clinical data.
- Probable: Good supporting evidence, though not definitively proven.
- Suspected: Some evidence suggests this interaction, but it is not well established.
- Possible: Limited or conflicting evidence; the interaction may occur.
- Theoretical: Predicted from the drugs' pharmacology; not yet confirmed in people.
Ratings come from the documented interaction literature and are reviewed by a pharmacist. They describe the documented risk of the combination, not what will necessarily happen to you — your dose, timing, and health picture all matter.
What happens
Increased haloperidol exposure and an increased risk of haloperidol-related adverse events, including QTc interval prolongation
Interaction Deep Dive
Coadministration of haloperidol (CYP3A substrate)1 and atorvastatin (weak CYP3A4 inhibitor) 2 may increase haloperidol exposure, which may increase haloperidol-related adverse events, including QTc interval prolongation. If concomitant use is required, monitor for signs or symptoms of increased or prolonged pharmacologic effects of haloperidol and reduce haloperidol dosage if necessary 1. Additionally, myalgia associated with coadministration of atorvastatin and haloperidol was described in a case report 3.
Why it happens (mechanism)
Inhibition of CYP3A4-mediated metabolism of haloperidol

a) Occurrences of adverse events attributed to drug interactions between antipsychotic drugs (used for schizophrenia or bipolar disorder) and drugs used for the treatment of cardiovascular diseases were evaluated in a study using data collected between January 2, 2017 to March 30, 2018 from the University Center for Clinical Drug Adverse Effects Monitoring and Study at the Clinic of Pharmacology of the Jagiellonian University Medical College (N=52 cases). The cause-effect relationship was either probable (n=41) or certain (n=11) for all evaluated cases within the study. Twelve cases reported interactions between antipsychotics and statins, with most describing signs or symptoms of muscle disorders. Specifically, 1 case described a 74-year-old female patient that experienced myalgia with coadministration of atorvastatin and haloperidol. The suspected mechanism was inhibition of atorvastatin metabolism 3.
