How we grade severity & evidence
Severity levels
- Contraindicated: These should generally not be used together.
- Major: Potentially serious — often needs a change or close monitoring.
- Moderate: Can be significant — usually manageable with monitoring.
- Minor: Usually limited clinical impact.
Evidence grades
- Established: Well documented — supported by controlled studies or strong clinical data.
- Probable: Good supporting evidence, though not definitively proven.
- Suspected: Some evidence suggests this interaction, but it is not well established.
- Possible: Limited or conflicting evidence; the interaction may occur.
- Theoretical: Predicted from the drugs' pharmacology; not yet confirmed in people.
Ratings come from the documented interaction literature and are reviewed by a pharmacist. They describe the documented risk of the combination, not what will necessarily happen to you — your dose, timing, and health picture all matter.
What happens
Decreased dolutegravir exposure with reduced efficacy
Interaction Deep Dive
Concomitant use of dolutegravir, a CYP3A substrate2, with nevirapine, a CYP3A inducer 1, may lead to decreased plasma concentrations and efficacy of dolutegravir 5. Coadministration should be avoided as data needed to make dosing recommendations are insufficient 342. If concomitant use is necessary, monitor for reduced dolutegravir efficacy and consider increasing the dose to keep dolutegravir trough concentration above 300 mcg/L 5.
Why it happens (mechanism)
Induction of CYP3A-mediated metabolism of dolutegravir by nevirapine

a) In a study of 10 patients with HIV, concurrent administration of dolutegravir with a nevirapine/abacavir/lamivudine regimen resulted in significant differences in dolutegravir pharmacokinetics compared with a regimen with abacavir/lamivudine only. There was a 19% decrease in dolutegravir AUC, a 34% decrease in dolutegravir trough concentration, a 15% decrease in dolutegravir half-life, and a 23% increase in dolutegravir oral clearance 5
