Reduced efficacy of oral cyclosporine

Concomitant use of oral cyclosporine and sevelamer may result in decreased plasma concentrations and reduced efficacy of cyclosporine. During postmarketing experience in transplant patients in whom the 2 drugs were coadministered, reduced cyclosporine concentrations leading to dose increases have been reported without any clinical consequences (ie, graft rejection). Thus, separation of the administration of the 2 drugs should be considered; the duration of separation depends upon the absorption characteristics (eg, Tmax and whether an immediate-release or extended-release formulation) of the coadministered medications. Consider monitoring clinical response or blood levels of cyclosporine1.
